摘  要:目的:探讨脱细胞血管基质修复胆管缺损的动物体内实验疗效。方法:将同种异体血管脱细胞,由此生成的人工胆管应用于贵州小香猪胆管缺损动物实验模型。术后不同时期(5周、4个月)检测相关指标,探讨人工胆管修复胆管缺损的疗效。结果:所有猪在整个研究期间均存活,未出现胆瘘或梗阻性黄疸表现。人工胆管在植入5周时排空通畅,未发现转氨酶升高,组织学检查发现人工胆管中有副腺样结构。植入4个月时肉眼不能区别人工胆管与原始胆管,胆管造影未发现管腔狭窄表现,血生化检查结果在正常范围。置入部位出现与原始胆管上皮类似的立方柱状上皮。结论:脱细胞化血管基质可能可以为临床胆管缺损或狭窄的治疗提供潜在的治疗途径。Objective:To investigate the effect of acellular vascular matrix on repairing bile duct defects in vivo.Methods:The artificial bile duct generated from the allogeneic blood vessel was applied to the experimental model of bile duct defect in 8 Guizhou Xiaoxiang pigs.The related indexes were detected at different postoperative periods(5 weeks and 4 months)to explore the efficacy of artificial bile duct in repairing bile duct defects.Results:All pigs survived throughout the study period without biliary fistula or obstructive jaundice.At 5 weeks after implantation,the emptying of artificial bile duct was smooth,and no elevation of transaminase was found.Histological examination showed that there were accessory adenoid structures in the artificial bile duct.At 4 months after implantation,artificial bile duct and primary bile duct could not be distinguished by naked eyes.No stricture of lumen was found by cholangiography,and the results of blood biochemical examination were in normal range.Cubic columnar epithelium similar to the primary bile duct epithelium appeared at the site of implantation.Conclusion:Acellular vascular matrix may provide a potential therapeutic approach for the treatment of bile duct defects or stricture.
