经皮冷冻球囊导管消融与射频导管消融治疗阵发性心房颤动的效果观察

摘　　要：目的对比分析经皮冷冻球囊导管消融(CBA)与射频导管消融(RFCA)治疗阵发性心房颤动(PAF)的临床效果。方法回顾性分析2017年1月-2018年1月收治的98例PAF患者的临床资料,根据治疗术式不同分为CBA组64例和RFCA组34例。观察2组手术情况,比较术中各支肺静脉隔离(PVI)成功率差异,观察术后并发症发生情况,记录术后3、6、9、12个月时复发情况。结果CBA组的手术时间、X线曝光时间、消融时间均短于RFCA组(P<0.01)。2组术中各支PVI成功率以及术后3、6、9、12个月时复发率比较差异均无统计学意义(P>0.05)。CBA组术后并发症总发生率低于RFCA组(P<0.05)。结论CBA与RFCA治疗PAF的临床效果相当,但CBA在改善手术情况和安全性方面有一定优势。Objective To compare efficacy of percutaneous cryoballoon ablation(CBA)and radiofrequency catheter ablation(RFCA)in treatment of patients with paroxysmal atrial fibrillation(PAF).Methods Clinical data of 98 PAF patients admitted during January 2017 and January 2018 was retrospectively analyzed,and the patients were divided into CBA group(n=64)and RFCA group(n=34)according to different therapeutic procedures.In two groups,operation conditions were observed,and difference in success rate of intraoperative each pulmonary vein isolation(PVI)was compared;incidence rates of postoperative complications were observed,and recurrence conditions in 3 rd,6 th,9 th and 12 th months after operation were recorded.Results Operative time,X-ray exposure time and ablation time in CBA group were significantly shorter than those in RFCA group(P<0.01).There were no significant differences in success rate of intraoperative each PVI and recurrence rates in 3 rd,6 th,9 th and 12 th months after operation between two groups(P>0.05).Incidence rate of postoperative complications in CBA group was significantly lower than that in RFCA group(P<0.05).Conclusion CBA has equivalent efficacy with RFCA in treatment of PAF patients,but CBA has a certain advantages in improving operation condition and safety.
