摘 要:目的探讨血糖波动与老年2型糖尿病患者并发认知功能障碍的关系。方法选择2015年1月-2018年12月收治的老年2型糖尿病患者100例。观察认知功能障碍发生情况。记录所有患者的相关临床资料,包括年龄、性别、体重指数、受教育年限、病程、吸烟史、血压、血脂、空腹血糖、糖化血红蛋白等。并采用CGMS动态血糖监测系统对患者进行72 h连续血糖监测。结果本组患者中发生认知功能障碍41例,发生率为41.00%。认知功能障碍组低密度脂蛋白胆固醇、空腹血糖、糖化血红蛋白、高血糖时间比、空腹血糖平均值、血糖波动系数、平均餐后血糖波动幅度、日内平均血糖波动幅度、血糖水平标准差及日间血糖波动幅度均高于非认知功能障碍组(P<0.05)。结论老年2型糖尿病患者并发认知功能障碍的发生率较高,且与血糖波动关系密切。Objective To investigate relationship between blood sugar fluctuation with cognitive dysfunction in elderly patients with type 2 diabetes mellitus.Methods Incidence conditions of 100 elderly patients with type 2 diabetes mellitus admitted during January 2015 and December 2018 were observed.Related informations of all patients such as age,gender,body mass index,education time,course of disease,smoking history,blood pressure,blood fat,fasting plasma glucose(FPG)and glycosylated hemoglobin were recorded,and CGMS dynamic blood glucose monitoring system was used for 72 h continuous blood glucose monitoring.Results A total of 41 patients had cognitive dysfunction,and incidence rate of cognitive dysfunction was 41.00%.Values of low density lipoprotein cholestorol(LDL-C),FPG,glycosylated hemoglobin(HbA1c),time ratio of hyperglycemia,FPG average value,fluctuation coefficient of blood glucose,average fluctuation amplitude of postprandial blood glucose,average fluctuation amplitude of intra-day blood glucose,standard deviation of blood glucose level and inter-day fluctuation amplitude of blood glucose in cognitive dysfunction group were significantly higher than those in non-cognitive dysfunction group(P<0.05).Conclusion Incidence rate of cognitive dysfunction in elderly patients with type 2 diabetes mellitus is higher,and it is closely related to fluctuation of blood glucose.
