摘　　要：目的:腹腔镜阑尾切除术对阑尾炎患者的疗效及对血清超敏C反应蛋白(hs-CRP)的影响。方法:选择2015年4月到2018年4月我院接诊的阑尾炎患者95例作为研究对象,以简单随机分组法分为观察组(n=50)和对照组(n=45),对照组使用开腹切除术,观察组采用腹腔镜切除术。比较两组手术时间、术后视觉模拟评分法(VAS)评分、住院时间、出血量、术后胃肠道恢复时间、hs-CRP水平、肿瘤坏死因子α(TNF-α)及术后不良反应发生情况。结果:观察组手术时间、术中出血量及术后VAS评分均显著低于对照组(P<0.05);治疗后,两组hs-CRP、TNF-α水平均明显改善;观察组优于对照组(P<0.05);观察组患者胃肠功能恢复时间均优于对照组(P<0.05);两组并发症总发生率为8.00%、28.89%,差异显著(P<0.05)。结论:在阑尾炎患者中应用腹腔镜阑尾切除术效果显著,对患者血清hs-CRP影响较小。Objective:To study the curative efficacy of laparoscopic appendectomy on appendicitis and its effectsonserum Hypersensitive c-reactive protein(hs-crp).Methods:95 cases of patients with appendicitis treated in our hospital from April 2015 to April 2018 were selected as study objects,The patients were divided into observation group(n=50)and control group(n=45)according to the simple random number table method.The control group received laparotomy,while the observation group received laparoscopic resection.Operation time,postoperative visual analogue scale(VAS)score,hospital stay,blood loss,postoperative gastrointestinal recovery time,hs-crp level,tumor necrosis factor alpha(tnf-alpha)and postoperative adverse reactions were compared between the two groups.Results:The operation time,intraoperative blood loss and postoperative VAS score of the observation group were significantly lower than those of the control group(P<0.05).After treatment,the levels of hs-crp and tnf-alpha increased significantly in both groups.The levels of hs-crp and TNF-alpha in the observation group were significantly lower than those in the control group(P<0.05).The recovery time of bowel sound,anal exhaust time,anal defecation time and hospitalization time in the observation group were significantly lower than those in the control group(P<0.05).The total incidence of complications in the observation group was 8.00%,significantly lower than that in the control group(P<0.05).Conclusion:the application of laparoscopic appendectomy in patients with appendicitis has a significant effect and a small effect on serum hs-crp,which is worthy of promotion and application.
