原发性膜性肾病患者炎性因子变化情况研究

摘　　要：目的 探讨原发性膜性肾病(PMN)患者血清炎性因子水平及治疗后炎性因子水平的变化情况。方法 选取2017年1月至2018年12月确诊的PMN患者200例及正常体检者100例。检测治疗前PMN患者确诊时及应用免疫抑制剂治疗后1个月、2个月后及对照组血清炎性因子白细胞介素-2(IL-2)、IL-4、IL-6、IL-10水平。结果 PMN患者血清IL-2、IL-4、IL-6水平显著高于对照组,IL-10水平显著低于对照组(P<0.05)。PLA2RAb阳性PMN患者血清炎性因子IL-2、IL-4、IL-6水平显著高于PLA2RAb阴性PMN患者,IL-10水平显著低于PLA2RAb阴性PMN患者(P<0.05)。规范治疗后,PLA2RAb阳性及PLA2RAb阴性PMN患者血清炎性因子L-2、IL-4、IL-6水平逐渐下降,IL-10水平逐渐升高(P<0.05),以PLA2RAb阳性PMN患者血清炎性因子水平变化更为明显。治疗后2个月,PLA2RAb阳性及PLA2RAb阴性PMN患者血清炎性因子水平差异无统计学意义(P>0.05),但仍高于正常对照组(P<0.05)。结论 PMN患者,尤其是PLA2RAb阳性PMN患者血清炎性因子变化明显,规范治疗能改善患者免疫状况。Objective To investigate the changes of inflammatory factors in patients with primary membranous nephropathy(PMN)before and after treatment.Methods A total of 200 patients with PMN,who were diagnosed and treated in our hospital from January 2017 to December 2018,were enrolled as research group,and 100 healthy subjects who underwent physical examination at the same period were enrolled as control group.The levels of serum interleukin(IL)-2,IL-4,IL-6 and IL-10 of the patients with PMN before treatment and at 1 month,2 month after treatment and those in control group were detected.Results The serum levels of IL-2,IL-4,IL-6 in research group were significantly higher than those in control group,however,the serum levels of IL-10 were significantly higher than those in control group(P<0.05).The serum levels of IL-2,IL-4 and IL-6 in PLA2RAb positive PMN patients were significantly higher than those in PLA2RAb negative PMN patients,however,the serum levels of IL-10 were significantly lower than those in PLA2RAb negative PMN patients(P<0.05).After treatment,the levels of serum L-2,IL-4 and IL-6 in PLA2RAb positive PMN patients and PLA2RAb negative PMN patients were decreased gradually,but the serum levels of IL-10 were increased gradually(P<0.05),in which the changes in PLA2RAb positive PMN patients were more significant.After 2-month treatment there were no significant differences in the levels of serum inflammatory factor between PLA2RAb positive PMN patients and PLA2RAb negative PMN patients,however,which were still significantly higher than those in control group(P<0.05).Conclusion The significant changes of the levels of serum inflammatory factors are found in PMN patients,especially in PLA2RAb-positive PMN patients.Standardized treatment can improve the immune status of patients.
