摘　　要：目的 探讨经直肠探头超声诊断肛瘘及其分型的临床价值。方法 采用直肠腔内探头超声诊断肛瘘Parks分型及内口、主管、支管结果与术后手术记录对照比较。结果 超声诊断括约肌间型、经括约肌型、括约肌外型、皮下或黏膜型、括约肌上型、复杂型的灵敏度分别为98.9%、97.8%、100%、95.8%,92.8%、83.8%。超声诊断肛瘘内口灵敏度为91.7%,主瘘管灵敏度为90.7%,支管灵敏度为67.8%。结论 经直肠超声对肛瘘Parks分型的诊断灵敏度较高,能较清晰的显示肛瘘管的部位、走行及内口位置,能为临床诊断及治疗提供较有价值的信息。Objective To investigate the clinical value of transrectal ultrasound in the diagnosis of anal fistula and its typing.Methods Transrectal ultrasonography was used to diagnose the Parks type of anal fistula,internal stoma,main fistul and branch fistula,which were compared with the postoperative diagnosis results.Results The sensitivity of ultrasonography in the diagnosis of inter-sphincter type,trans-sphincter type,external sphincter type,subcutaneous or mucosal type,sphincter superior type and complex type anal fistula was 98.9%,97.8%,100%,95.8%,92.8%and 83.8%,respectively.The sensitivity of ultrasonography in the diagnosis of internal stoma of anal fistula.the main fistula and the branch fistula was 91.7%,90.7%and 67.8%,respectively.Conclusion The transrectal ultrasound has a high diagnostic sensitivity for Parks typing of anal fistula,which can clearly display the position,course of anal fistula,and provide valuable information for clinical diagnosis and treatment.
