血清补体与IgA、IgM、IgG联合检测在肺炎支原体肺炎诊断中的价值研究

摘　　要：目的 研究血清补体与IgA、IgM、IgG联合检测在肺炎支原体肺炎疾病诊断中的价值。方法 选取2017年2月至2018年2月小儿肺炎患儿100例为疾病组,根据肺炎患儿氧合标准,将疾病组分为普通组(n=67)和重症组(n=33),另选健康人群100例作为健康组,分别对2组患儿的血清补体及IgA、IgM、IgG水平进行对比观察,分析不同病变程度患儿的血清补体及IgA、IgM、IgG水平之间的差异,同时分析联合检测血清补体及IgA、IgM、IgG水平对支原体肺炎的诊断意义。结果 疾病组患儿的血清补体C3、C4、IgM、IgG水平显著高于健康组(P<0.05),IgA水平显著低于健康组(P<0.05),重症组患儿的血清补体C3、C4、IgM、IgG水平显著高于普通组(P<0.05),IgA水平显著低于普通组(P<0.05);血清补体及IgA、IgM、IgG水平均可作为肺炎的独立危险因素;普通肺炎和重症肺炎患儿的补体C3临界值分别为1.78、1.92 g/L,补体C4临界值分别为0.42、0.61 g/L,IgA临界值分别为1.27、1.11 g/L,IgG临界值分别为8.32、11.33 g/L,IgM临界值分别为1.67、1.88 g/L,通过联合检测和单独检测分析,普通肺炎和重症肺炎患儿的联合检测患儿的曲线下面积分别高于单独检测。结论 血清补体与IgA、IgM、IgG联合检测,对于患儿的免疫功能具有良好的预测作用,对患儿的治疗效果疾病请评估准确度较好,建议在临床工作中进行推广。Objective To investigate the application value of combination detection of serum complement,IgA,IgM and IgG in diagnosis of mycoplasma pneumoniae pneumonia.Methods A total of 100 children patients with pneumonia who were treated in our hospital from February 2017 to February 2018 were enrolled as case group,according to the oxygenation standard of pneumonia,the case group was subdivided into common case group(n=67)and servere case group(n=33),at the same time,100 healthy subjects were enrolled as control group.The serum levels of complements,IgA,IgM and IgG were observed and compared between the two groups.The differences of serum levels of complements and IgA,IgM and IgG in children patients with different degrees of pathological changes were analyzed.Meanwhile the diagnostic significance of combination detection of serum complements and the levels of IgA,IgM and IgG in diagnosis of mycoplasma pneumonia was analyzed.Results The serum levels of complement C3,C4,IgM and IgG in case group were significantly higher than those in control group(P<0.05),and the IgA levels in case group were significantly lower than those in control group(P<0.05).Moreover the serum levels of complement C3,C4,IgM and IgG in severe case group were significantly higher than those in common case group(P<0.05),and the IgA levels in severe case group were significantly lower than those in common case group(P<0.05).The serum complement and the levels of IgA,IgM and IgG could be regarded as independent risk factors of pneumonia.The critical value of complement C3 in common case group and severe case group was 1.78g/L and 1.92g/L,respectively,and the critical value of complement C4 was 0.42g/L and 0.61g/L,respectively,and the critical value of IgA was 1.27g/L and 1.11g/L,respectively,and the critical value of IgG was 8.32g/L and 11.33g/L,respectively,the critical value of IgM was 1.67g/L and 1.88g/L,respectively.The area under the curve(AUC)of combination detection in common case group and severe case group was significantly higher than that by
