摘　　要：目的研究无缝隙护理对子宫内膜癌合并糖尿病患者化疗效果的影响。方法选取接受治疗的子宫内膜癌合并糖尿病并采用CAP化疗方案(环磷酰胺500 mg/m^2,阿霉素40 mg/m^2,顺铂70 mg/m^2)治疗的88例患者作为研究对象,采用随机数字表法将其分为A(传统护理)、B(无缝隙护理)2组。观察2组患者化疗效果和3年内生存曲线;遵医行为和自我护理行为的改善情况;治疗后1年内不良事件发生情况和生活质量评分。结果2组患者化疗有效率A组显著低于B组,差异有统计学意义(Z=5.147,P=0.023)。3年的随访,B组患者存活率显著高于A组,差异有统计学意义(χ^2=4.919,P=0.027)。B组在干预后1个月和3个月时各项依从性指标显著高于A组同时期评分,差异有统计学意义(P<0.05)。A组患者口腔溃疡和感染发生人数显著多于B组,差异有统计学意义(P<0.05)。SF-36量表组间评分结果显示,除了在维度躯体疼痛上2组间差异无统计学意义(P>0.05),在其他项目上B组得分均高于A组,差异有统计学意义(P<0.05)。结论无缝隙护理应用于子宫内膜癌合并糖尿病患者护理效果良好,能够显著提高患者化疗有效率、降低病死率、改善遵医行为、减低不良事件发生率和提高生活质量。Objective To investigate the effects of seamless nursing on the chemotherapy effects on endometrial cancer complicated by diabetes mellitus.Methods A total of 88 patients with endometrial cancer complicated by diabetes mellitus who were treated in our hospital were enrolled in the study.All the patients were treated by CAP chemotherapy regimen(cyclophosphamide 500mg/m^2+doxorubicin 40mg/m^2+cisplatin 70mg/m^2),then these patients were randomly divided into two groups:group A(traditional nursing)and group B(seamless nursing).The chemotherapy effects and 3-year survival curve,the improvement status of compliance behavior and self-care behavior as well as the incidence of adverse events and life quality scores within 1 year after treatment were observed and compared between the wto groups.Results The chemotherapy efficacy in group A was significantly lower than that in group B(P<0.05).After 3-year follow-up,the survival rate in group B was significantly higher than that in group A(P<0.05).Moreover the compliance indexes at 1m and 3m after intervention in group B were significantly higher than those in group A(P<0.05).The incidence rates of oral ulcers and infections in group A were significantly higher than those in group B(P<0.05).The results of the SF-36 scale showed that except for the dimension body pain scores,the scores in other indexes in group B were significantly higher than those in group A(P<0.05).Conclusion The seamless nursing in treatment of endometrial cancer complicated by diabetes mellitus can obtain satisfactory effects,which can significantly improve the chemotherapy effects,reduce the fatality rate,improve the compliance behavior,decrease the incidence of adverse events and improve the life quality of patients.
