摘 要:目的探讨银杏叶注射液对CO中毒迟发性脑病(DEACMP)患者脑功能及磁共振成像(MRI)的影响。方法收治该院DEACMP患者86例,随机分为观察组与对照组,每组43例。对照组常规治疗,观察组在对照组基础上加用银杏叶注射液治疗30 d。比较两组血清中神经元特异性烯醇化酶(NSE)、丙二醛(MDA)及超氧化物歧化酶(SOD),蒙特利尔认知评估量表(MoCA)、Barthel指数(BI)、简易精神状态检查量表(MMSE)评分及MRI检查神经功能。结果观察组总有效率明显高于对照组(P<0.05)。治疗后两组MRI异常信号强度减弱,观察组重度病变率(3级强度)明显低于对照组(P<0.05)。治疗后两组MDA、NSE明显低于治疗前,SOD活力明显高于治疗前(P<0.05);观察组MDA、NSE水平明显低于对照组,SOD活力明显高于对照组(P<0.05)。治疗后两组MoCA、MMSE及BI评分明显高于治疗前,观察组MoCA、MMSE及BI评分明显高于对照组(P<0.05)。结论银杏叶注射液可有效改善DEACMP患者神经功能及MRI异常表现。Objective To investigate the effect of ginkgo biloba injection on brain function and magnetic resonance imaging(MRI) in patients with delayed encephalopathy(DEACMP)caused by CO poisoning. Methods 86 DEACMP patients admitted to our hospital were randomly divided into observation group and control group,43 patients in each group. The control group received conventional treatment,and the observation group was treated with ginkgo biloba injection for 30 d on the basis of the control group. Neuronal specific enolase(NSE),malondialdehyde(MDA)and superoxide dismutase(SOD),Montreal cognitive assessment scale(MoCA),Barthel index(BI),simple mental state examination scale(MMSE)and MRI were compared between the two groups. Results The total effective rate in the observation group was significantly higher than that in the control group(P<0.05). After treatment,the intensity of MRI abnormal signals in the two groups was reduced,and the rate of severe lesions in the observation group(Category 3 strength)was significantly lower than that in the control group(P<0.05). After treatment,MDA and NSE in the two groups were significantly lower than that before treatment,and SOD activity in the two groups was significantly higher than that before treatment(P<0.05). MDA and NSE levels in the observation group were significantly lower than those in the control group,and SOD activity was significantly higher than that in the control group(P<0.05). After treatment,MoCA,MMSE and BI scores in the two groups were significantly higher than those before treatment in same groups,and MoCA,MMSE and BI scores in the observation group were significantly higher than those in the control group(P<0.05). Conclusions Ginkgo biloba injection effectively improved the neurological function and MRI abnormalities of DEACMP patients.
