摘  要:目的探讨铅暴露对7~12岁儿童甲状腺激素及神经系统的影响。方法根据血铅水平将368名7~12岁儿童分为观察组(48例)和对照组(320例)。对两组进行甲状腺激素、血清生长激素(GH)、生长激素结合蛋白(GHBP)、胰岛素样生长因子-1(IGF-1)、胰岛素样生长因子结合蛋白-3(IGFBP-3)、神经元特异性烯醇化酶(NSE)检测,并对两组进行体格发育水平和智力水平检测。结果观察组促甲状腺素(TSH)水平明显高于对照组,游离甲状腺素(FT4)/TSH、NSE水平均明显低于对照组(P<0.05)。两组总三碘甲腺原氨酸(TT3)、总甲状腺素(TT4)、游离三碘甲腺原氨酸(FT3)、FT4水平比较,差异无统计学意义(P>0.05)。观察组GH、IGF-1、IGFGP-3水平均明显低于对照组(P<0.05);两组GHBP水平比较,差异无统计学意义(P>0.05)。观察组与对照组的身高、体质量、胸围、肺活量指标比较,差异有统计学意义(P<0.05)。对照组智力发育水平明显优于观察组(P<0.05)。结论铅暴露影响儿童甲状腺激素水平、智力发育及体格发育相关的激素。Objective To investigate the effects of lead exposure on thyroid hormone and nervous system in children aged 7~12 years. Methods According to blood lead level,368 children aged 7~12 years were divided into observation group(48 cases)and control group(320 cases). Thyroid hormone,serum growth hormone(GH),growth hormone binding protein(GHBP),insulin-like growth factor-1(IGF-1),insulin-like growth factor-binding protein-3(IGFGP-3)and neuron-specific enolase(NSE)were detected in the two groups,and physical development level and intelligence level were detected in the two groups. Results The levels of thyrotropin(TSH)and free thyroxin(FT4)/TSH and NSE in the observation group were significantly higher than those in the control group(P<0.05). There was no statistically significant difference between the two groups in the levels of total triiodothyroid acid(TT3),total thyroxine(TT4),free triiodothyroid acid(FT3)and FT4(P>0.05). GH,IGF-1 and IGFGP-3 levels in the observation group were significantly lower than those in the control group(P<0.05). There was no significant difference in GHBP level between the two groups(P>0.05). There were statistically significant differences in height,body mass,chest circumference and vital capacity between the observation group and the control group(P<0.05). The intelligence development level of the control group was significantly better than that of the observation group(P<0.05). Conclusions Lead exposure affects thyroid hormone levels,mental and physical development in children.
