丹参酮胶囊治疗顽固性湿疹的疗效与β防御素-2 mRNA表达的关系

摘　　要：目的:探究丹参酮胶囊治疗顽固性湿疹的疗效及其对人体β防御素-2 mRNA表达的影响。方法:选择2016年9月至2018年5月于我院就诊的顽固性湿疹患者98例,随机分两组,每组49例。对照组口服盐酸西替利嗪片,外用艾洛松乳膏。观察组患者在此基础上,加用丹参酮胶囊。8周疗程结束后分析治疗效果,检测两组的HβD-2 mRNA的表达情况及L-2、IL-4、IFN-γ等细胞因子水平。结果:治疗后,两组患者的ENSI评分和瘙痒感评分显著低于治疗前,观察组显著低于对照组(P<0.05);观察组的治疗效率93.88%显著高于对照组的73.47%(P<0.05);观察组的不良反应发生率8.16%与对照组的6.12%比较,无显著差异(P>0.05);治疗前后,两组患者非皮损组织处的β防御素-2 mRNA表达无明显差异(P>0.05)。治疗后,观察组皮损组织的β防御素-2 mRNA表达显著低于对照组(P<0.05);治疗后,观察组患者IL-2、IFN-γ水平显著低于对照组,而IL-4水平显著高于对照组(P<0.05)。结论:丹参酮胶囊用于治疗顽固性湿疹,见效快,有效降低皮损处的β防御素-2 mRNA表达水平,调整Th1/Th2细胞因子水平,有助于缓解临床症状,安全可靠、复发率低,值得进一步研究。Objective:To explore the efficacy of tanshinone capsules in the treatment of refractory eczema and its effect on the expression of HβD-2 mRNA.Methods:98 patients with refractory eczema who were treated in our hospital from September 2016 to May 2018 were enrolled.According to the random number method,the patients were divided into observation group(n=49)and control group(n=49).The control group received oral cetirizine hydrochloride tablets and topical ilosin cream.On the basis of this,patients in the observation group were given tanshinone capsules.The therapeutic effect was analyzed after the end of the 8-week treatment period,and the expression of HβD-2 mRNA and cytokine levels such as L-2,IL-4 and IFN-γwere detected in the two groups.Results:After treatment,the ENSI score and pruritus score of the two groups were significantly lower than those before treatment,and the observation group was significantly lower than the control group(P<0.05).The treatment efficiency of the observation group was 93.88%significantly higher than that of the control group(73.47%)(P<0.05);the incidence of adverse reactions in the observation group was 8.16%compared with 6.12%in the control group,there was no significant difference(P>0.05).Before and after treatment,the expression ofβ-defensin-2 mRNA in non-lesional tissues of the two groups was not obvious.Difference(P>0.05).After treatment,the expression ofβ-defensin-2 mRNA in the lesions of the observation group was significantly lower than that in the control group(P<0.05).After treatment,the levels of IL-2 and IFN-γin the observation group were significantly lower than those in the control group,while IL-4 levels were significantly higher than the control group(P<0.05).Conclusion:Tanshinone capsule is used for the treatment of refractory eczema.It has a quick effect and effectively reduces the expression ofβ-defensin-2 mRNA,adjusts the level of Th1/Th2 cytokines,which helps to relieve clinical symptoms,is safe and reliable,and has a low recurrence rate.The method deserves
