斑秃患者血清25-羟维生素D3水平检测及卡泊三醇治疗的效果评价

摘　　要：目的:探讨斑秃患者血清25-羟维生素D3水平及卡泊三醇局部治疗的临床效果。方法:选取笔者医院收治的92例斑秃患者和60例健康志愿者,比较两组血清25-羟维生素D3水平。将斑秃患者中血清25-羟维生素D3水平正常和低下者分为正常组和低下组,斑秃患者中根据治疗方式不同又随机分成联合组和对照组。其中对照组患者口服甘草酸苷制剂,联合组在此基础上联合卡泊三醇搽剂局部治疗。治疗疗程均为12周,每2~4周复诊1次,观察患者治疗效果和局部不良反应等情况。结果:斑秃患者血清25-羟维生素D3水平显著低于健康志愿组(P<0.05)。接受卡泊三醇治疗其临床有效率显著高于未接受卡泊三醇者(P<0.05)。结论:血清25-羟维生素D3水平可能参与斑秃发病机制,卡泊三醇治疗斑秃有效,其疗效在血清25-羟维生素D3水平正常和低下患者中无明显差异。Objective To investigate the serum level of 25-hydroxyvitamin D3 and the clinical efficacy of topical calcipotriol in the patients with alopecia areata.Methods A total of 92 patients with alopecia areata and 60 healthy controls were enrolled to compare the serum 25-hydroxyvitamin D3 levels in our hospital.The patients with alopecia areata were divided into normal group and lower group according to serum 25-hydroxyvitamin D3 level.They were randomly divided into combined group and control group,respectively.In control group,the patients were only treated with oral compound glycyrrhizin capsules,and the patients in combined group were treated with oral compound glycyrrhizin capsules combined with calcipotriol liniment.The course of treatment was 12 weeks,and the patient was revisited once every 2 or 4 weeks.The treatment efficiency and adverse reactions were observed respectively.Results The level of serum 25-hydroxyvitamin D3 in patients with alopecia areata were significantly lower than those in healthy controls(P<0.05).The clinical efficacy of treatment with calcipotriol was significantly higher than that of patients without calcipotriol(P<0.05).Conclusion The level of serum 25-hydroxyvitamin D3 may be involved in the pathogenesis of alopecia areata.Calcipotriol is effective in the treatment of alopecia areata,which has no significant difference between normal and low level of serum 25-hydroxyvitamin D3.
