髂内动脉结扎联合纱布填塞术治疗骨盆骨折大出血患者的临床观察

摘　　要：目的:分析髂内动脉结扎联合纱布填塞术治疗骨盆骨折大出血的临床效果。方法:选自我院2017年1月-2018年12月所收治的20例骨盆骨折大出血患者,根据不同的止血方式分为两组,其中对照组10例(髂内动脉结扎联合纱布填塞术),控制组10例(导管动脉栓塞术)。本研究对两组术前、术后的临床观察进行对比分析。结果:对照组患者休克控制时间、输血量低于控制组;术后ICU住院时间、骨折平均愈合时间、感染发生率均高于控制组(P<0.05)。结论:髂内动脉结扎联合纱布填塞术适用于临床治疗骨盆骨折大出血,止血效果较好,但易发生感染;导管动脉栓塞术虽然较好地减少发生感染的可能性,但专业性要求强,临床操作难度大。Objective:To analyze the clinical effect of internal iliac artery ligation combined with gauze packing in the treatment of massive hemorrhage caused by pelvic fracture.Methods:20 cases of pelvic fracture hemorrhage admitted to our hospital from January 2017 to December 2018 were selected and divided into two groups according to different hemostasis methods,including 10 cases in the control group(internal iliac artery ligation combined with gauze packing)and 10 cases in the treatment group(catheter arterial embolization).In this study,the preoperative and postoperative clinical observations of these two groups of patients were compared and analyzed.Results:The shock control time in the control group were lower than those in the treatment group.Postoperative ICU hospitalization time,mean fracture healing time and infection rate were all higher than those in the treatment group(P<0.05).Conclusion:Internal iliac artery ligation combined with gauze packing is suitable for the clinical treatment of massive hemorrhage caused by pelvic fracture.Although catheter arterial embolization can reduce the possibility of infection,it is very difficult to operate in clinic because of its high professional requirement.
