摘 要:目的探讨电针人中、百会对脑缺血再灌注大鼠BBB损伤的作用机理,为针刺治疗脑卒中提供科学依据。方法选用健康雄性SD大鼠,随机分为正常对照组、模型组和电针组,采用Longa线栓改良法建立大鼠脑缺血再灌注(MCAO)模型,电针组在造模成功后电针刺激人中、百会30 min。采用神经功能缺损评分、CV染色法测定脑水肿肿胀率、免疫组织化学法与Western blot方法检测MCAO大鼠脑组织中ZO-1表达及以透射电镜观察大鼠右侧脑组织纹状体组织超微结构的变化。结果随着缺血再灌注时间的延长,大鼠神经功能缺损及BBB损伤程度均随再灌注时间延长而逐渐加重,而电针组的大鼠神经功能缺损程度明显低于模型组;模型组大鼠在缺血再灌注6 h缺血侧脑半球开始肿胀,并于72 h达到高峰,电针组大鼠在缺血再灌注24 h、48 h、72 h与模型组比较,差异有显著性(P <0.05或P <0.01);随着缺血再灌注时间的延长,模型组ZO-1蛋白表达显著降低,显著低于电针组,在缺血再灌注24 h、48 h和72 h经统计学处理,有显著性差异(P <0.05);电镜图示电针组较模型组脑组织超微结构损伤较小。结论电针人中、百会可减轻脑缺血再灌注造成的神经损伤,下调脑水肿肿胀率,抑制ZO-1蛋白下降,保护脑组织内的超微结构,以抑制脑缺血再灌注后血脑屏障的损伤。Objective This thesis discussed the mechanism of electroacupuncture at philtrum and Baihui points against BBB injury in rats with cerebral ischemia-reperfusion, and provided a scientific basis for acupuncture treatment of stroke. Methods Healthy male SD rats were randomly divided into the normal control group, the model group and the electroacupuncture group. Rat model of middle cerebral artery occlusion(MCAO) was established by modified thread-occlusion Longa method. The electroacupuncture group stimulated philtrum and Baihui acupoints for 30 minutes after successfully modeling.The cerebral edema swelling rate was measured by neurological deficit score and CV staining method. The expression of ZO-1 in brain tissue of MCAO rats was detected by immunohistochemistry method and Western blot method, and the ultrastructural changes of striatum tissue in right brain tissue of rats were observed by transmission electron microscope. Results With the extension of the reperfusion time, the degree of neurological deficit and BBB injury in rats increased gradually with the extension of the reperfusion time, while the degree of neurological deficit in rats in electroacupuncture group was significantly lower than that in model group. The cerebral hemisphere of the ischemic side of the model group began to swell at 6 h after ischemia reperfusion and reached its peak at 72 h. The rats in the electroacupuncture group were significantly different from the model group at 24 h, 48 h, and 72 h after ischemia-reperfusion(P < 0.05, P < 0.01). With the extension of the ischemia reperfusion time, the expression of ZO-1 protein in the model group was significantly lower than that in the electroacupuncture group. After statistical treatment at 24 h, 48 h and 72 h of ischemia reperfusion, there was a significant difference(P < 0.05). Electron microscopy showed that the electroacupuncture group had less damage to the ultrastructure of the brain tissue than the model group.
