摘　　要：目的:总结临床药师参与耐碳青霉烯类肺炎克雷伯菌(CRKP)抗感染治疗的经验和体会。方法:在1例老年男性CRKP尿路感染治疗中,临床药师参与治疗方案的制订,包括阿米卡星个体化剂量调整和聚维酮碘膀胱冲洗,并在治疗过程中做好药学监护。结果:临床药师利用自身专业知识协助临床医师优化给药方案,取得良好的抗感染治疗效果。结论:临床药师参与抗感染治疗方案的制定与调整,可加强抗菌药物的合理应用和减少相关药品不良反应的发生。Objective:To summarize the experience of clinical pharmacists participated in the treatment of carbapenem resistant Klebsiella pneumoniae(CRKP).Methods:Clinical pharmacists participated in the formulation of the treatment plan in the treatment of an elderly man with urinary tract infection caused by CRKP,including the individualized dose adjustment of amikacin and povidone iodine bladder irrigation and pharmaceutical monitoring.Results:Clinical pharmacists assisted clinicians in optimizing dosage regimen by using their professional knowledge,by which some good anti-infection treatment results were obtained.Conclusion:The assistance of clinical pharmacists in the formulation and adjustment of anti-infective therapy can strengthen the rational application of antimicrobial agents and reduce the occurrence of adverse drug reactions.
