化痰活血方治疗稳定型心绞痛痰阻血瘀型疗效及对一氧化氮、内皮素-1、血脂的影响

摘　　要：目的观察化痰活血方治疗稳定型心绞痛痰阻血瘀型患者的临床疗效及对一氧化氮(NO)、内皮素-1(ET-1)、血脂的影响。方法将100例稳定型心绞痛痰阻血瘀型患者随机分为2组,每组50例。对照组给予常规西医治疗,观察组在对照组治疗基础上加服化痰活血方,2组疗程均为4周。统计2组心绞痛症状和心电图疗效,观察2组治疗后心绞痛发作频率、心绞痛发作时间、硝酸甘油用量的变化,检测2组治疗前后血清NO、ET-1及血脂指标水平。结果观察组治疗后心绞痛症状总有效率及心电图总有效率均明显高于对照组(P均<0.05);2组治疗后心绞痛发作频率均明显降低(P均<0.05),心绞痛发作时间明显缩短(P均<0.05),硝酸甘油用量明显减少(P均<0.05),观察组治疗后心绞痛发作改善情况明显优于对照组(P均<0.05),且硝酸甘油用量明显少于对照组(P均<0.05);2组治疗后血清ET-1水平及三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)水平均明显降低(P均<0.05),NO水平及高密度脂蛋白胆固醇(HDL-C)水均明显升高(P均<0.05),观察组治疗后以上指标改善情况均明显优于对照组(P均<0.05)。结论在西医治疗稳定型心绞痛的基础上,加用化痰活血方能够提高临床疗效,明显减少硝酸甘油用量,其机制可能与改善血管内皮功能和血脂紊乱有关。Objective It is to observe the clinical efficacy of Huatan Huoxue decoction in the treatment of patients with stable angina pectoris of type of phlegm-blood stasis,and its effects on nitric oxide(NO),endothelin-1(ET-1)and blood lipids.Methods One hundred patients with stable angina pectoris of type of phlegm-blood stasis were randomly divided into two groups,50 cases in each group.The control group was treated with conventional western medicine.The observation group was treated with Huatan Huoxue decoction on the basis of the control group.Both groups were treated for 4 weeks.The curative effect of symptoms of angina pectoris and electrocardiogram were evaluated.The changes of frequency of angina pectoris,duration of angina pectoris and the dosage of nitroglycerin after treatment were observed.The levels of serum NO,ET-1 and serum lipids were measured before and after treatment.Results The total effective rates of symptoms of angina pectoris and ECG were significantly higher in the observation group than that in the control group(P<0.05).The frequency of angina pectoris was significantly decreased,and the duration of angina pectoris was significantly shortened(P<0.05),the dosage of nitroglycerin was significantly reduced after treatment in the two groups(P<0.05).The improvement of angina pectoris in the observation group was significantly better,and the dosage of nitroglycerin was significantly less than that of the control group(P<0.05).The levels of serum ET-1,triglyceride(TG),total cholesterol(TC),and low-density lipoprotein cholesterol(LDL-C)after treatment were significantly lower while that of high density lipoprotein cholesterol(HDL-C)was significantly higher than those before treatment in the two groups(P<0.05),the improvement of the above indicators in the observation group was significantly better than that of the control group(P<0.05).Conclusion On the basis of western medicine for the treatment of stable angina pectoris,the addition of Huatan Huoxue decoction can improve the clinical efficacy and sign
