摘  要:目的分析miR-133a、miR-133b在急性心肌梗死(AMI)患者血浆中的表达,并探讨miR-133a、miR-133b在诊断AMI中的应用价值。方法选取2017年1月至2017年12月于西安交通大学第一附属医院胸痛中心就诊的AMI患者为观察组(n=89),同期就诊于西安交通大学第一附属医院体检中心的既往无心脏病史,心电图正常的对照人群37例为正常对照组(n=37)。收集所有受试者的临床资料并提取血浆标本,实时定量PCR法检测循环血浆中miR-133a及miR-133b的表达水平。应用受试者工作特征曲线ROC评估对AMI的辅助诊断价值。结果AMI患者血浆中miR-133a及miR-133b的含量较正常对照组显著升高,差异均有统计学意义(P<0.05);受试者工作特征曲线(ROC)分析表明:miR-133a的ROC曲线下面积AUC值为0.845(P<0.01),敏感度为84.5%,特异性为69.0%,漏诊率为15.5%,误诊率31.0%;miR-133b的ROC曲线下面积AUC值为0.702(P<0.01),敏感度为70.2%,特异性为51.7%,漏诊率为29.8%,误诊率为48.3%。结论循环miR-133a及miR-133b在诊断AMI方面具有较好的特异性和敏感性,可作为AMI诊断的生物学标志物。Objective To analyze the expression of miR-133a and miR-133b in plasma of patients with acute myocardial infarction(AMI),and to explore the value of miR-133a and miR-133b in the diagnosis of AMI.Methods From January 2017 to December 2017,the AMI patients were selected as observation group(n=89),and they were treated at the first affiliated hospital of Xi'an Jiaotong University.A total of 37 patients in the control group with normal medical history and electrocardiogram were normal controls(n=37).The clinical data of all subjects were collected and plasma samples were taken.Real-time quantitative PCR was used to detect the expression levels of miR-133a and miR-133b in circulating plasma.The area under the receiver operating characteristic curve(AUC)was used to reflect the diagnostic value.Results The levels of miR-133a and miR-133b in plasma of AMI patients were significantly higher than those in the normal control group(P<0.05).he AUC of miR-133a was 0.845(P<0.01),the sensitivity was 84.5%,the specificity was 69.0%,the missed diagnosis rate was 15.5%,and the misdiagnosis rate was 31.0%.The AUC of miR-133b was 0.702(P<0.01),the sensitivity was 70.2%,the specificity was 51.7%,the missed diagnosis rate was 29.8%,and the misdiagnosis rate was 48.3%.Conclusion Circulating miR-133a and miR-133b have good specificity and sensitivity in the diagnosis of AMI,and can be used as a biomarker for the diagnosis of AMI.
