膝关节镜下钛缆内固定治疗青少年胫骨前髁间棘骨折的临床研究

摘　　要：目的探讨膝关节镜下钛缆内固定方法治疗青少年胫骨前间棘骨折的临床疗效。方法选取2016年5月-2018年12月创伤骨科胫骨前髁间棘骨折患者18例,均采用膝关节镜下钛缆内固定治疗。根据Meyers-Mckeever分型:Ⅱ型8例,Ⅲ型7例,Ⅳ型3例。术后记录患者膝关节活动度,术后4周、8周、12周行前抽屉试验、Lachman试验,并对患侧与健侧进行Lysholm评分。结果18例患者均完成随访,术后4周随访,X线片显示:均未出现骨折移位或不愈合及固定失效;前抽屉试验、Lachman试验均为阴性。术后8周随访,患者患侧膝关节Lysholm评分低于健侧(t=-9.103,P<0.05)。术后12周随访,患者患侧膝关节Lysholm评分均为优,与健侧差异无统计学意义(t=-1.458,P>0.05)。结论关节镜下钛缆内固定治疗胫骨前髁间骨折,手术创伤小,膝关节功能恢复较好,是一种有效的治疗方法。Objective To investigate the clinical effect of titanium cable fixation under knee arthroscopy in the anterior intercondylar spine fracture of tibia.Methods From May 2016 to December 2018,total of 18 cases of adolescents with anterior intercondylar spine fracture of tibia were treated with titanium cable fixation under knee arthroscopy,including 12 males and 6 females.According to Meyers-Mckeever classification,8 cases were of typeⅡ,7 cases were of typeⅢ,and 3 cases were of typeⅣ.The range of motion of knee joint was recorded after operation.The anterior drawer test and Lachman test were performed 8 and 12 weeks after operation,and Lysholm scores were performed on the affected side and the healthy side.Results All 18 patients completed the follow-up.The X-ray films showed that there was no fracture displacement or nonunion and fixation failure.The anterior drawer test and Lachman test were negative.The Lysholm score of the affected knee was lower than that of the healthy knee(t=-9.103,P<0.05).At 12 weeks follow-up,the Lysholm score of the affected knee joint was excellent,and there was no significant difference between the two groups(t=-1.458,P>0.05).Conclusion It is an effective method to treat tibial anterior intercondylar fracture with titanium cable under arthroscope,with small surgical injury and better knee function.
