摘  要:目的:探讨三维(3D)打印联合数字化设计在复杂胫骨平台骨折内固定治疗中的应用效果。方法:选择94例复杂胫骨平台骨折患者,根据就诊单双日将其分为对照组(43例)和观察组(51例)。对照组接受传统手术治疗,观察组接受3D打印联合数字化设计辅助手术治疗,比较两组手术切开显露时间、复位固定时间、术中出血量、Rasmussen膝关节功能评分和美国特种外科医院(HSS)膝关节临床功能评分情况。结果:观察组手术切开显露时间及复位固定时间较对照组显著缩短,差异有统计学意义(t=2.030,t=4.519;P<0.05);观察组术中出血量少于对照组,差异有统计学意义(t=6.976,P<0.05);术后3 d,观察组骨折复位优良率高于对照组,差异有统计学意义(x^2=4.573,P<0.05);术后6个月观察组HSS膝关节功能评分高于对照组,差异有统计学意义(t=3.136,P<0.05)。结论:3D打印联合数字化设计应用于复杂胫骨平台骨折内固定治疗中,能够较好地缩短手术时间、减少术中出血量,提高手术质量,并加快术后康复进程。Objective:To explore the application effect that 3D printing combined with digital design in internal fixation of the fractures of complex tibial plateau.Methods:A total of 94 patients with fractures of complex tibial plateau who were treated in the hospital were enrolled.They were divided into control group(43 cases)and observation group(51 cases)according to even-odd day of registration.The control group underwent traditional surgery treatment,and the observation group underwent surgery treatment that was assisted by 3D printing with digital design.The exposure time,the time of reduction and fixation,the amount of intraoperative blood loss,Rasmussen knee-joint function score and HSS knee-joint clinical function score between the two groups were compared.Results:The exposure time of surgery incision and the time of reduction and fixation in the observation group were significantly shorter than those in control group(t=2.030,t=4.519,P<0.05).The amount of intraoperative blood loss of observation group was significantly lower than that of control group(t=6.976,P<0.05).At the 3th day post operation,the excellent rate of fracture reduction in observation group was significantly higher than that in control group(x^2=4.573,P<0.05).At 6 month post operation,the HSS knee-joint function score of observation group was significantly higher than that of control group(t=3.136,P<0.05).Conclusion:The application of 3D printing with digital design in internal fixation of the fractures of complex tibial plateau can relatively better shorten operation time,reduce the amount of intraoperative blood loss,improve quality of surgery and accelerate postoperative rehabilitation process.
