摘 要:目的比较革兰氏阳性细菌与革兰氏阴性细菌所致脓毒症大鼠心肌损伤的异同。方法雄性SD大鼠45只,随机分为革兰氏阳性细菌脓毒症组、革兰氏阴性细菌脓毒症组及生理盐水对照组,各组大鼠分别予以腹腔注射3 ml热灭活金黄色葡萄球菌(7.35×10^12 CFU/Kg)、3 ml热灭活大肠埃希菌(1.26×10^12 CFU/Kg)或3 ml生理盐水。建模成功后6、12和24 h分别于大鼠尾静脉采血,采用全自动化学发光免疫分析仪检测血浆心肌钙蛋白(cardiac troponin,cTnT)、肌酸激酶(creatine kinase,CK)和肌酸激酶同工酶(creatine kinase-MB,CK-MB)表达。结果与生理盐水对照组比较,革兰氏阳性细菌脓毒症组与革兰氏阴性细菌脓毒症组血浆心肌酶水平均随时间延长逐渐升高,差异有统计学意义(P<0.05);与革兰氏阴性细菌脓毒症组比较,革兰氏阳性细菌脓毒症组大鼠血浆心肌酶水平明显升高,两组间差值亦随时间延长而增加,24 h时达到峰值,差异均有统计学意义(P<0.05)。结论与革兰氏阴性菌所致脓毒症大鼠心肌损伤相比,革兰氏阳性细菌所致脓毒症大鼠心肌损伤程度更为严重。Objective The objective of this study was to analyze the changes in myocardial enzymes in rats with sepsis induced by Gram-positive or Gram-negative bacteria at different stages of myocardial injury.Methods 45 male SD rats were randomly divided into Gram-positive bacterial sepsis group,Gram-negative bacterial sepsis group,and saline group.Inactivated Staphylococcus aureus(7.35×10^12 CFU/Kg,3 ml),inactivated Escherichia coli(1.26×10^12 CFU/Kg,3 ml)and saline solution(3 ml)were used to establish the models by intraperitoneal injection.Blood was collected at 6,12 and 24 hours after successful modeling from tail vein of rats.Plasma cardiac enzymes(cTnT,CK,CK-MB)were measured by automatic chemiluminescence immunoassay.Results Compared with saline group,the values of plasma myocardial enzymes in Gram-positive bacterial sepsis group and Gram-negative bacterial sepsis group increased over time,peaking at 24 hours(P<0.05).Compared with Gram-negative bacterial sepsis group,the value of myocardial enzyme in Gram-positive bacterial sepsis group increased significantly,and the difference between the two groups increased gradually over time,peaking at 24 hours(P<0.05).Conclusions Compared with Gram-negative bacterial sepsis group,the severity of myocardial injury in Gram-positive bacterial sepsis rats is more severe.
