MRI和超声在剖宫产子宫瘢痕妊娠诊断中的应用对比分析

摘　　要：目的探讨3.0T MR与超声检查在诊断子宫瘢痕妊娠(Cesarean scar pregnancy,CSP)中的价值。方法回顾性收集我院临床疑诊并手术治疗的CSP患者29例,所有患者术前均经MRI、超声检查,采用配对卡方检验比较两种检查方法诊断结果有无统计学差异,取P<0.05为差异具有统计学意义。以手术病理为标准,比较两种检查方法灵敏度、特异度、漏诊率、符合率等。结果磁共振与超声诊断瘢痕妊娠结果无统计学差异性(χ2=0.25,P=0.63),磁共振诊断瘢痕妊娠灵敏度及诊断符合率均高于超声,灵敏度分别为92.6%和85.2%,诊断符合率分别为93.1%和86.2%,漏诊率低于超声分别为7.4%和14.8%,特异度均为100%。结论MRI在诊断早期子宫瘢痕妊娠中具有较高的诊断价值,可作为超声检查的有效补充。Objective To explore the value of 3.0 T MR and ultrasonography in diagnosing cesarean scar pregnancy(CSP).Methods 29 cases of CSP patients with suspected clinical diagnosis and surgical treatment were performed magnetic resonance imaging(MRI)and ultrasound.The two methods of inspection diagnosis were compared by paired chi-square test(P<0.05 was set as statistical significance).The sensitivity,specificity,missed diagnosis rate and coincidence rate of the two methods were compared according to the criteria of surgical pathology.Results There was no statistically significant difference between MRI and ultrasonic diagnosis of scar pregnancy(χ^2=0.25,P=0.63).The sensitivity and diagnostic conformance rate of MRI diagnosis of scar pregnancy were higher than that of ultrasound.The sensitivities were 92.6%and 85.2%,respectively.And the diagnostic conformance rates were 93.1%and 86.2%,respectively.The rates of missed diagnosis were 7.4%and 14.8%,and the specificities were both 100%.Conclusion MRI has high diagnostic value in the diagnosis of early uterine scar pregnancy and can be used as an effective supplement to ultrasound examination.
