低剂量盐酸右美托咪定改善重症监护室老年患者睡眠质量的研究

摘　　要：目的观察低剂量右美托咪定对重症监护室(ICU)老年患者睡眠质量的影响,为改善患者的睡眠质量提供方法。方法选取2017年11月—2018年11月在成都市青白江区人民医院ICU住院的96例老年患者为研究对象,采用随机数字表法分为观察组(n=48)和对照组(n=48)。观察组给予低剂量右美托咪定治疗,对照组给予咪达唑仑治疗。评估并比较2组睡眠结构、意识指数、睡眠质量、APACHEⅡ评分和ICU停留时间等情况。结果治疗后,观察组N1期睡眠比、N3期睡眠比、NREM期睡眠比、REM期睡眠比和睡眠效率均显著高于对照组,差异均有统计学意义(P<0.05);观察组觉醒指数显著低于对照组(P<0.05)。观察组APACHEⅡ评分和ICU停留时间均明显少于对照组(P<0.05)。结论低剂量盐酸右美托咪定可改善ICU老年患者睡眠质量,有利于促进患者预后,具有一定的临床应用价值。Objective To investigate the effect of low-dose dexmedetomidine on the sleep quality of the elderly in intensive care unit(ICU).Methods 96 elderly in ICU during the period from November,2017 to November,2018 were randomly divided into 2 groups:observation group(n=48)and control group(n=48);low-dose dexmedetomidine was administrated to the elderly in observation group and midazolamto the elderly in control group;a comparative study was made to the sleep structure,index of consciousness,sleeping quality,APACHEⅡscores,and the time-length of ICU staying between the 2 groups.Results The ratios of N1 phase,N3 phase,NREM phase,REM phase and the sleep efficiency in observation group after treatment were statistically higher while the arousal index much lower in observation group after treatment than those in control group(P<0.05).Conclusions Low-dose dexmedetomidine can improve the sleeping quality and promote the prognosis of the elderly in ICU,which is worthy of clinical application.
