活血化瘀类中药注射剂治疗缺血性中风的临床效果及不良反应评价

摘　　要：目的比较常用的5种活血化瘀类中药注射剂治疗缺血性中风的临床效果及不良反应。方法收集2018年1—12月收治的缺血性中风患者1000例为研究对象,根据使用药物不同分为血栓通组(注射用血栓通)、灯盏细辛组(灯盏细辛注射液)、丹红组(丹红注射液)、疏血通组(疏血通注射液)、银杏内酯组(银杏内酯注射液),每组200例。比较分析这5组的治疗效果及不良反应评价。结果血栓通组、灯盏细辛组、丹红组、疏血通组、银杏内酯组在经过治疗后,美国国立卫生研究院卒中量表(NIHSS)评分、Barthel指数、血液流变学相关指标均得到明显改善(P<0.05),但是丹红组NIHSS评分、Barthel指数、血液流变学相关指标的改善程度最好,总有效率最高,银杏内酯组不良反应的发生率最高(P<0.05)。在不良反应类别中,说明书外的不良反应占每组不良反应人数的比例较大,疏血通组不良反应发生的主要年龄段在61~70岁,其他4组的主要年龄段均在80岁以上。结论5种活血化瘀类中药注射剂中丹红注射液的治疗效果最好,且不良反应较少。在临床上应根据患者个体化差异合理应用活血化瘀类药物,尽可能降低不良反应发生率。Objective To compare efficacy and adverse reactions of five kinds of commonly used activating blood circulation to dissipate blood stasis TCM injections in treatment of patients with ischemic stroke.Methods A total of 1000 patients with ischemic stroke admitted from January to December 2018 were divided into Xueshuantong group(Xueshuantong injection,n=200),Dengzhanxixin group(Dengzhanxixin injection,n=200),Danhong group(Danhong injection,n=200),Shuxuetong group(Shuxuetong injection,n=200)and Ginkgolide group(Ginkgolide injection,n=200)according to the different medications.Therapeutic effects and conditions of adverse reactions were compared in five groups.Results After treatment,values of national institute of health stroke scale(NIHSS)score,Barthel indexes and related hemorheology indexes in Xueshuantong,Dengzhanxixin Danhong,Shuxuetong and Ginkgolide groups were significantly improved compared with those before treatment(P<0.05);in Danxi group,improved degrees of NIHSS score,Barthel indexes and related hemorheology indexes were the best,and the total effective rate was the highest,while the highest incidence rate of adverse reactions was found in Ginkgolide group(P<0.05).In category of adverse reaction,adverse reactions without mentioning in instructions accounted for a large proportion of the number of adverse reactions in each group.Main age group of adverse reactions was found in 61-70 years old patients in Shuxuetong group,while the main age of adverse reactions in the other 4 groups were all 80 years old.Conclusion Among the five kinds of activating blood circulation to dissipate blood stasis TCM injections,Danhong injection has the best therapeutic effect and less adverse reactions.Activating blood circulation to dissipate blood stasis drugs should be rational application according to individualized differences of patients in clinic so as to reduce incidence rate of adverse reactions as much as possible.
