摘 要:目的分析糖皮质激素抵抗性哮喘患者支气管黏膜中Raf-1、Mek1/2、Erk1/2的表达水平,探讨其对糖皮质激素抵抗性哮喘的预测和诊治的意义。方法选取2015年1月至2018年1月在商洛市中心医院呼吸内科治疗的糖皮质激素抵抗性哮喘50例作为研究组,另选取同期健康体检人员50例为对照组,利用肺功能测试仪检测两组受试者第1秒最大肺活量(FEV1)、最大呼吸气流速(PEF)、最大通气量(MVV),采用蛋白免疫印迹法(WB)检测Raf-1、Mek1/2及Erk1/2在两组支气管黏膜中的表达情况,利用ROC曲线分析Raf-1、Mek1/2和Erk1/2对糖皮质激素抵抗性哮喘的诊断价值,并采用非条件Logistic多元回归分析法分析影响糖皮质激素抵抗性哮喘发生的危险因素。结果研究组患者FEV1、PEF和MVV分别为(1.23±0.18)L、(3.15±0.42)L/s、(46.05±9.29)L/min,均明显低于对照组的(1.52±0.41)L、(5.72±1.69)L/s、(59.27±14.16)L/min,差异均有统计学意义(P<0.05);研究组患者支气管黏膜中的Raf-1、Mek1/2和Erk1/2表达量分别为0.87±0.07、0.65±0.16和0.69±0.12,均明显高于对照组的0.45±0.08、0.37±0.05、0.34±0.04,差异均有统计学意义(P<0.05);ROC曲线分析结果显示,Raf-1评估诊断糖皮质激素抵抗性哮喘的价值相对较高;Logistic多元回归分析结果显示,FEV1、Raf-1和Erk1/2是导致糖皮质激素抵抗性哮喘发生的危险因素(P<0.05)。结论糖皮质激素抵抗性哮喘患者支气管黏膜中Raf-1、Mek1/2、Erk1/2呈异常高表达,Raf-1可以作为糖皮质激素抵抗性哮喘的判断指标,为临床上预测及诊治提供一定的参考依据。Objective To analyze the expression levels of Raf-1,Mek-1/2 and Erk-1/2 in bronchial mucosa of patients with glucocorticoid-resistant asthma,and to explore their significance in the prediction,diagnosis and treatment of glucocorticoid-resistant asthma.Methods From January 2015 to January 2018,50 patients with glucocorticoid-resistant asthma treated in the Department of Respiratory Medicine at Shangluo Central Hospital were selected as the study group,and 50 healthy physical examination personnel were selected as the control group.Pulmonary function tester was used to detect the forced expiratory volume in 1 s(FEV1),peak expiratory flow(PEF)and maximal voluntary ventilation(MVV)in both groups.Western blotting(WB)was used to detect the expression of Raf-1,Mek1/2 and Erk1/2 in bronchial mucosa of the two groups.The diagnostic value of Raf-1,Mek1/2 and Erk1/2 for glucocorticoid-resistant asthma was analyzed by ROC curve,and the risk factors affecting the occurrence of glucocorticoid-resistant asthma were analyzed by unconditional logistic multiple regression analysis.Results The FEV1,PEF and MVV of the study group were(1.23±0.18)L,(3.15±0.42)L/s,and(46.05±9.29)L/min,respectively,which were significantly lower than corresponding(1.52±0.41)L,(5.72±1.69)L/s,(59.27±14.16)L/min of the control group(P<0.05).The expression levels of Raf-1,Mek1/2 and Erk1/2 in the bronchial mucosa of the study group were 0.87±0.07,0.65±0.16 and 0.69±0.12 respectively,which were significantly higher than corresponding 0.45±0.08,0.37±0.05,0.34±0.04 of the control group(P<0.05).The results of ROC curve analysis showed that Raf-1 has a relatively high value in the diagnosis of glucocorticoid-resistant asthma.Logistic multiple regression analysis showed that FEV1,Raf-1 and Erk1/2 were risk factors for glucocorticoid-resistant asthma(P<0.05).Conclusion The expressions of Raf-1,Mek1/2 and Erk1/2 in bronchial mucosa of patients with glucocorticoid-resistant asthma show abnormally high expression,and Raf-1 can be used as a predictor of gl
