自体骨联合BMP-2治疗骨折术后骨不连的疗效比较

摘　　要：目的比较自体骨联合骨形态发生蛋白-2(BMP-2)与单纯自体骨移植治疗骨折术后骨不连的疗效。方法将骨折术后骨不连的42例患者随机分为自体骨联合BMP-2治疗组(实验组,22例)和单纯自体骨移植治疗组(对照组,20例),比较两组的骨折愈合及患肢功能情况。结果42例术后随访6?24个月,平均随访(11.27±4.18)个月。两组均取得较好的临床疗效,但实验组的术后骨痂出现时间、第3、6个月时骨痂形成评分、骨折愈合时间和愈合率、以及末次随访时的患肢功能评级均优于观察组(P<0.05)。结论自体骨移植联合BMP-2治疗骨折术后骨不连的效果优于单纯自体骨移植治疗,可促进骨折愈合,改善患肢功能。Objective To compare the effects between autogenous bone graft combined with the bone morphogenetic protein-2(BMP-2)and autogenous bone graft alone in the treatment of postoperative bone nonunion.Methods A total of 42 patients with postoperative bone nonunion were randomly divided to an experiment group(n=22)and a control group(n=20),with the former being treated with autogenous bone graft combined with BMP-2 and the latter with autogenous bone graft alone.The fracture union and affected limb function were compared.Results All patients were followed up for six to 24 months,with an average of(11.27±4.18)months.A satisfactory clinical outcome was achieved for both groups.The time to callus formation,scores of callus formation at the third and sixth months,fracture union time and union rate,and the function grading of the affected limb at the last follow-up in the experiment group were all superior to the control group(P<0.05).Conclusion Autogenous bone graft combined with BMP-2 is preferable to the autogenous bone graft alone in the treatment of postoperative bone nonunion,for it performs better in promoting bone union and lifting affected limb function.
