摘　　要：目的探讨临床药师参与低分子肝素抗凝引起血小板减少症患者的药学监护的方法。方法临床药师对一例肝小静脉闭塞症患者使用低分子肝素抗凝后血小板计数降低的案例进行分析,讨论HIT的治疗和监护。结果临床药师参与治疗过程,调整抗凝药物后患者血小板恢复正常水平。结论临床药师参与治疗过程,为临床提供药学服务,保障患者抗凝药物用药安全。Objective To explore the method of clinical pharmacists participating in pharmaceutical care for patients with thrombocytopenia caused by anticoagulation of low molecular weight heparin.Methods Clinical pharmacists analyzed a case of thrombocytopenia in a patient with hepatic veno-occlusive disease after anticoagulation with low molecular weight heparin,and discussed the treatment and monitoring of HIT.Results Clinical pharmacists participate in the treatment process,adjust the anticoagulant drugs to restore the normal level of platelets in patients.Conclusion Clinical pharmacists participate in the treatment process,provide pharmaceutical care for the clinic,and ensure the safety of anticoagulant drugs used by patients.
