摘 要:目的研究右美托咪啶对肝移植患者围术期脑能保护作用产生的影响,为后期治疗该疾病提供依据。方法选取我院在2017年3月~2019年3月期间收治的76例行同种异体肝移植患者,采用抛硬币的方法随机分为研究组和对照组,每组患者各38例,对照组患者静脉滴注负荷量为1μg/kg生理盐水,滴注时间为麻醉诱导前10 min,完成滴注后泵注0.5μg/kg.h至手术完成,研究组患者采用相同体积的右美托咪啶替代生理盐水。研究比较两组患者无肝期、新肝期脑氧摄取率,比较两组患者术后智能精神状态评分(MMSE)。结果研究组患者术后MMSE评分高于对照组,数据差异显著(P<0.05),研究组患者无肝期和新肝期脑氧摄取率均低于对照组,数据差异显著(P<0.05)。结论右美托咪啶能够减少肝移植患者脑氧摄取率,改善患者精神状态,对围术期脑功能保护作用显著,值得临床推广。Objective To study the effect of dexmedetomidine on perioperative brain energy protection in patients with liver transplantation,and to provide evidence for the later treatment of the disease.Methods Sixtysix patients with allogeneic liver transplantation who were admitted to our hospital from March 2017 to March 30,19 were randomly divided into study group and control group,with 38 cases in each group.The intravenous infusion load of the patients was 1μg.kg-1 saline,the infusion time was 10 minutes before the induction of anesthesia,and 0.5μg.kg.h-1 was pumped after the completion of the instillation.The patients in the study group used the same volume.Dexmedetomidine replaces saline.The study compared the cerebral oxygen uptake rate between the two groups in the period of no liver and new liver,and compared the postoperative mental state score(MMSE)between the two groups.Results The postoperative MMSE scores of the study group were higher than those of the control group.The data were significantly different(P<0.05).The cerebral oxygen uptake rate of the study group was lower than that of the control group.The data were significantly different(P<0.05).Conclusion Dexmedetomidine can reduce the cerebral oxygen uptake rate of patients with liver transplantation,improve the mental state of patients,and has a significant protective effect on perioperative brain function,which is worthy of clinical promotion.
