运用云医疗技术提高维持性血液透析患者自我管理能力的研究

摘　　要：目的探讨运用云医疗技术对提高维持性血液透析患者自我管理能力的作用和影响。方法选取2017年11月至2018年5月在重庆某三级甲等医院血液透析中心实行血液透析的患者80例,将其随机分为对照组(40例)和观察组(40例)。对照组采用传统的健康宣教模式,观察组在传统宣教模式基础上,增加基于云计算、3G通信以及多媒体等云医疗技术的健康宣教模式。干预前,两组患者都填写个人一般资料调查问卷,并首次完成血液透析患者自我管理量表测评;干预6个月后,两组患者再次完成自我管理量表测评。比较干预前后两组的自我管理评分差异。结果干预前,两组患者的自我管理水平总评分,差异较小无统计学意义(P>0.05);干预后,组间比较观察组自我管理总评分以及执行自我护理、伙伴关系、问题解决、情绪处理四维度分项评分均高于对照组,差异有统计学意义(P<0.05)。组内比较两组自我管理总评分及四维度分项评分,干预后均高于干预前,差异有统计学意义(P<0.05)。结论运用云医疗技术可以提高健康宣教的效率,对提升维持性血透患者自我管理能力产生积极影响。Objective To explore the effects of the Cloud Medical Treatment(CMT)technology on improving self-management skills of maintenance hemodialysis(MHD)patients.Methods A total of 80 patients with MHD in a tertiary hospital in Chongqing were recruited and divided equally and randomly into two groups-a control group(40 cases)and an observation group(40 cases).While patients in the observation group has received the routine nursing education,patients in the control group has received a more modernized way of nursing education supported by the Cloud Medical Treatment(CMT)technology,such as cloud computing,3G communication and multi-media.Both of the two groups were asked to finish the general information questionnaire and the Hemodialysis Self-Management Instrument(HD-SMI)for the first time before intervention.After six months of the experiment,both of the two groups were asked to finish the HD-SMI again.Self-management skills of the two groups assessed by the HD-SMI were compared before and after intervention.Results The total scores of self-management before intervention were compared between the two groups,and the difference was not statistically significant(P>0.05).The total score of self-management and each dimensionscore-self-care,partnership,problem-solving,emotion management in the control group were higher than those in observation group after intervention(P<0.05).Intra-group comparison:all dimension scores,as well as the total score,of each group were significantly higher after intervention(P<0.05).Conclusion The Cloud Medical Treatment(CMT)technology has greatly improved the nursing education efficiency,it has a remarkable positive impact on improving MHD patients’self-management skills.
