摘  要:目的观察正常听力者短音诱发的听性脑干反应(Tone-pip evoked auditory brainstem response,TP-ABR)低频波形分化特征与纯音听阈的相关性,为设立本实验室正常校正因子提供参考。并将其应用于低频听力损失的患者,探讨TP-ABR预估低频听力损失的可靠性。方法对20例听力正常人(40耳)和19例低频听力损失患者(19耳)进行TP-ABR测试。采用SPSS19.0软件对测量数据进行统计学分析。计量资料以x±s表示。两组间差异比较采用配对t检验,相关性比较采用线性回归分析。结果①正常受试者0.5、1、2、4kHz TP-ABR反应阈分别较对应频率纯音听阈高20.00±5.90,16.76±5.49,13.82±5.78,11.62±5.33dB;②正常组TP-ABR 0.5-4kHz V波潜伏期分别为11.85±0.79,10.89±1.17,9.58±0.86,8.60±0.67ms;③低频听力损失患者组和正常人组的TP-ABR反应阈与纯音听阈的校正值之间的差异<5dB,没有临床意义;④低频听力损失患者的TP-ABR减推荐校正因子后获得的预估听力级与其实际纯音听阈的线性回归分析显示,0.5、1kHz处两者的拟合度R2分别为0.787和0.797,相关性较好。结论通过TP-ABR反应阈值减推荐校正因子获得的预估听力级可以客观可靠的预估低频的听力情况。Objective To observe the waveform and correlation of low frequency tone pip evoked auditory brainstem response and provide reference for setting up the normal correction factor of laboratory.Then apply it to patients with low-frequency hearing loss.Analyze the reliability of low-frequency hearing thresholds.Methods 20 adults with normal hearing and 19 patients with low frequency hearing loss were included in this TP-ABR test.SPSS19.0 software was used to analyze the measured data.The measurement data is expressed as x±s.Comparison between the two groups using paired t test,correlation comparison using linear regression analysis.Results 1)Normal person’s response thresholds of 0.5,1,2 and 4 kHz TP-ABR were 20.00±5.90,16.76±5.49,13.82±5.78 and 11.62±5.33dB;2)The V-wave latency of 0.5-4kHz TP-ABR were 11.85±0.79,10.89±1.17,9.58±0.86,8.60±0.67ms,3)Linear regression analysis of predictive hearing level and pure tone hearing threshold in patients with low-frequency hearing loss shows the fitting degree R2 at 0.5 and 1 kHz is 0.787 and 0.797.4)The difference about correction factor between patients with low-frequency hearing loss and normal people don’t have clinical meaning.Conclusion The predictive hearing level obtained by TP-ABR minus correction factor,can be objective and reliable prediction of low frequency hearing.
