动脉内膜剥脱联合腰交感神经节切除术治疗下肢血栓闭塞性脉管炎的回顾性分析

摘　　要：目的探讨动脉内膜剥脱术联合腰交感神经节切除术对下肢血栓闭塞性脉管炎(TAO)的疗效。方法回顾性分析2010年7月至2018年7月哈尔滨医科大学附属第一医院行动脉内膜剥脱联合腰交感神经节切除术的108例(114条肢体)TAO患者的临床资料,比较踝肱指数(ABI)、保肢率、Rutherford分级和视觉模拟评分(VAS)以评价手术疗效。结果患者术后1周累计保肢率为100%,术后6个月累计保肢率为98.2%,术后12个月累计保肢率为95.6%。和术前相比,患者术后1周、6、12个月ABI均明显升高(P<0.05),Rutherford分级和VAS疼痛评分均明显降低(P<0.05)。结论动脉内膜剥脱术联合腰交感神经节切除术对TAO患者具有良好的效果。Objective To investigate the outcomes of endarterectomy combined with lumbar sympathectomy for thromboangiitis obliterans(TAO).Methods The clinical data of 108 patients(114 limbs)with TAO who underwent endarterectomy combined with lumbar sympathectomy in the First Affiliated Hospital of Harbin Medical University from July 2010 to July 2018 were retrospectively analyzed.Ankle brachial index(ABI),limb salvage rate,Rutherford classification and vsual analogue scale(VAS)were evaluated for the efficacy of surgery.Results The cumulative rate of limb salvage in 1 week,6 months and 12 months after operation were 100%,98.2%and 95.6%,respectively.ABI values were higher in 1 week,6 months and 12 months after operation than before operation(P<0.05),while Rutherford classification value and VAS were lower(P<0.05).Conclusion Endarterectomy combined with lumbar sympathectomy is an effective surgical method for TAO.
