摘  要:目的:探讨新生儿呼吸窘迫综合征应用高频振荡通气治疗的效果及对患者氧合指数的影响。方法:选取2013年6月-2017年6月本院收治的新生儿呼吸窘迫综合征患者82例。按照随机数字表法将其分为对照组和研究组,各41例。对照组应用高频振荡通气进行治疗,研究组应用高频振荡通气联合一氧化氮吸入并配合西地那非进行治疗。比较两组动脉血气分析指标(PaO2、PA-aDO2、PaO2/FiO2)、心肺功能指标(PAP、LVEF)、脑尿钠肽(BNP)、低氧诱导因子-1α(HIF-1α)、内皮素-1(ET-1)、用氧时间、机械通气时间、住院时间及不良反应情况。结果:研究组PaO2、PaO2/FiO2水平均高于对照组,PA-aDO2水平低于对照组,差异均有统计学意义(P<0.05);研究组LVEF水平高于对照组,PAP水平低于对照组,差异均有统计学意义(P<0.05);研究组BNP、HIF-1α、ET-1水平均低于对照组(P<0.05);研究组用氧时间、机械通气时间、住院时间均短于对照组(P<0.05);两组均未出现任何不良反应。结论:在针对新生儿呼吸窘迫综合征的治疗过程当中,应用高频振荡通气联合一氧化氮吸入并配合西地那非治疗的效果理想,临床上应当进一步推广应用。Objective:To explore the effect of high frequency oscillatory ventilation on neonatal respiratory distress syndrome and its influence on oxygenation index.Method:82 cases of neonatal respiratory distress syndrome admitted to our hospital from June 2013 to June 2017 were selected.According to the random number table method,they were divided into control group and study group,41 cases in each group.The control group was treated with high frequency oscillatory ventilation,study group was treated with high frequency oscillatory ventilation combined with nitric oxide inhalation and Sildenafil.The arterial blood gas analysis indexes(PaO2,PA-a DO2,Pa O2/Fi O2),cardiopulmonary function indexes(PAP,LVEF),brain natriuretic peptide(BNP),hypoxia inducible factor-1α(HIF-1α),endothelin-1(ET-1),oxygen use time,mechanical ventilation time,hospitalization time and adverse reactions between two groups were compared.Result:The levels of PaO2 and PaO2/Fi O2 in study group were higher than those of control group,and the levels of PA-aDO2 was lower than that of control group,the differences were statistically significant(P<0.05).The level of LVEF in study group was higher than that of control group,and the level of PAP was lower than that of control group,the differences were statistically significant(P<0.05).The levels of BNP,HIF-1αand ET-1 in study group were lower than those of control group(P<0.05).Oxygen use time,mechanical ventilation time and hospitalization time in study group were shorter than those of control group(P<0.05).There were no adverse reactions in both groups.Conclusion:In the treatment of neonatal respiratory distress syndrome,high frequency oscillatory ventilation combined with inhalation of nitric oxide and Sildenafil are effective,and should be further promoted and applied in clinic.
