摘 要:目的应用二维斑点追踪成像(2D-STI)评价无排异心脏移植患者的左室收缩功能。方法选取在我院行心脏移植手术后长期临床随访未发现明显排异反应的患者50例(心脏移植组)和同期健康志愿者30例(对照组),应用2D-STI检查获取两组左室心肌内膜、心肌外膜、整体纵向应变(endoGLS、epiGLS、GLS)、环向应变(endoGCS、epiGCS、GCS)及左室整体径向应变(G RS),并对两组进行比较分析。结果心脏移植组纵向应变参数GLS、endoGLS、epiGLS分别为(-16.02±2.95%)、(-18.75±3.44)%、(-15.41±2.46)%,环向应变参数endoGCS、GCS、epiGCS分别为(-25.74±4.59)%、(-15.22±3.79)%、(-10.66±3.73)%,径向应变参数GRS为(26.62±6.11)%,均较对照组减低,差异均有统计学意义(均P<0.001)。结论心脏移植患者即使未发生排异反应,其左室收缩功能仍较正常成人低。2D-STI是评价心脏移植患者左室收缩功能的有效手段。Objective To evaluate left ventricular systolic function in patients with non-rejection heart transplantation by two-dimensional speckle tracking imaging(2D-STI).Methods A total of 50 patients with no significant rejection after long-term clinical follow-up in our hospital were enrolled as heart transplantation group,and 30 healthy volunteers matched by age and sex at the same time as control group.Two-dimensional echocardiography was performed.The strain parameters analyzed by EchoPAC analysis software,including global longitudinal strain(GLS),GLS of endocardial and epicardial layers(endoGLS and epiGLS),global circumferential strain(GCS),GCS of endocardial and epicardial layers(endoGCS and epiGCS),global radial strain(GRS).The results of two groups were compared and analyzed.Results The longitudinal strain parameters GLS,endoGLS and epiGLS of the heart transplantation group were(-16.02±2.95)%,(-18.75±3.44)%,(-15.41±2.46)%,and the hoop strain parameters endoGCS,GCS and epiGCS were(-25.74±4.59)%,(-15.22±3.79)%,(-10.66±3.73)%,and the radial strain parameter GRS was(26.62±6.11)%,which were lower than that in the control group,and the difference were statistically significan(t all P<0.001).Conclusion Patients with heart transplantation have lower left ventricular systolic function than that of normal adults even without rejection.2D-STI technology is an effective method to evaluate left ventricular systolic function in heart transplant patients.
