彩超联合小切口治疗老年大隐静脉曲张所致溃疡的临床疗效观察

摘　　要：目的:观察彩超联合小切口治疗老年大隐静脉曲张所致溃疡与传统方式之间在麻醉要求、手术时间、出血量、治疗效果之间的差异。方法:选取2016年4月—2018年10月我院收治的老年大隐静脉曲张溃疡91例(104条患肢),随机分为治疗组和对照组,治疗组52例(61条患肢),对照组39例(43条患肢),治疗组采用局麻下彩超联合小切口进行处理,对照组采用椎管内麻醉下行大隐静脉高位结扎+剥脱术+交通静脉结扎术,进行手术时间、出血量、术后愈合情况对比。结果:治疗组患者手术时间及出血量均低于对照组,术后平均愈合时间及愈合率相比较差异均无统计学意义。结论:彩超联合小切口治疗老年大隐静脉曲张所致溃疡可以降低麻醉风险,缩短手术时间,创伤小,手术范围小,治疗效果相当。Objective:To observe the differences between color doppler ultrasound with small incisions used to treat the ulcer caused by varicose vein of elderly people and the traditional ways in terms of the anesthesia requirements,operation time,bleeding volume and therapeutic effect.Methods:Patients with ulcer caused by varicose vein of elderly people who had been received in our hospital from April 2016 to October 2018 are randomly divided into the treatment group and control group by random number table.There are 52 patients in the treatment group(61 limbs with ulcer)and 39 patients in the control group(43 limbs with ulcer).The treatment group was treated with local anesthesia with color doppler ultrasound with small incisions,and the control group underwent spinal anesthesia for high saphenous vein ligation+exfoliation+traffic vein ligation.Then all the patients in the treatment group and control group are compared in operation time,bleeding volume and postoperative healing situation.Results:The operation time and bleeding volume of patients in the treatment group are lower than that in the control group.There was no significant difference in mean healing time and healing rate between the two groups.Conclusion:Color doppler ultrasound with small incisions used to treat the ulcer caused by varicose vein of elderly people will reduce the risk of anesthesia and shorten the operative time with smaller trauma and scope of operation,as well as better treatment effect.
