摘 要:目的比较对硝基苯甲酸/噻吩-2-羧酸肼生长(PNB/TCH)和DNA微阵列基因芯片鉴定非结核分枝杆菌(NTM)临床分离株菌种的价值。方法收集2013年1月~2018年6月我院住院患者的痰分枝杆菌培阳分离菌2175株,分别采用PNB/TCH生长试验和DNA微阵列基因芯片鉴别NTM,比较两种方法的鉴定结果。结果PNB/TCH生长试验初步鉴定NTM 93株,占4.28%,DNA微阵列基因芯片鉴定NTM 79株,占3.63%,PNB/TCH生长试验初步鉴定的93株NTM中,有14例为假NTM(15.05%);PNB/TCH生长试验鉴定NTM的阳性检出率低于DNA微阵列基因芯片;79株NTM中,占比前3位的依次为胞内分枝杆菌株(37.97%)、堪萨斯分枝杆菌(21.52%)、鸟分枝杆菌(16.46%)。结论传统对硝基苯甲酸/噻吩-2-羧酸肼生长试验鉴定NTM特异性较低,采用DNA微阵列基因芯片技术鉴定NTM准确,临床参考价值较大。Objective To compare the growth of non-tuberculous mycobacteria(NTM)clinical isolates with p-nitrobenzoic acid/thiophene-2-carboxylate growth(PNB/TCH)and DNA microarray microarray.Methods A total of 2175 strains of Mycobacterium phlei isolates in our hospital from January 2013 to June 2018 were collected.PNB/TCH growth test and DNA microarray gene chip were used to identify NTM.Identification of the method.Results PNB/TCH growth assay initially identified NTM 93 strain,accounting for 4.28%,DNA microarray gene chip identified NTM 79 strain,accounting for 3.63%,and of the 93 NTMs initially identified by PNB/TCH growth test,14 were pseudo NTM(15.05%);PNB/TCH growth test identified that the positive detection rate of NTM was lower than that of DNA microarray gene chip;among the 79 strains of NTM,the top 3 were followed by intracellular branching strain(37.97%),Kansas Mycobacterium(21.52%),Mycobacterium avium(16.46%).Conclusion The traditional p-nitrobenzoic acid/thiophene-2-carboxylic acid oxime growth test has low specificity of NTM.The DNA microarray gene chip technology is used to identify NTM accurately,and the clinical reference value is large.
