针刀松解定点旋转体位下阳性反应点治疗青年神经根型颈椎病的临床研究

摘　　要：目的观察针刀松解定点旋转体位下阳性反应点治疗青年神经根型颈椎病的临床疗效。方法将60例青年神经根型颈椎病患者随机分为治疗组与对照组,每组30例。治疗组予定点旋转体位下阳性点针刀松解疗法,对照组予常规体位针刀松解疗法。两组疗程均为10日,观察临床疗效,比较症状体征评分及疼痛VAS评分的变化情况。结果①治疗组、对照组总有效率分别为90.00%和86.67%;组间临床疗效比较,差异无统计学意义(P>0.05)。②组间治疗5日后、治疗10日后比较,治疗组症状体征评分均高于对照组,疼痛VAS评分均低于对照组(P<0.05)。结论与传统体位相比,定点旋转体位下针刀松解治疗青年神经根型颈椎病,总体疗效相当,但此新方法便于寻找进针点,而且进针浅、安全系数高、对正常软组织损伤较轻,因而患者更易接受。Objective To observe the clinical efficacy in treatment of cervical spondylotic radiculopathy in young people with needle knife loosening therapy at positive reaction points at fixed-point rotating position. Methods Sixty young patients with cervical spondylotic radiculopathy were randomly assigned into treatment group and control group, 30 patients in each group. The treatment group was treated with needle knife loosening therapy at positive reaction points at fixed-point rotating position, while the control group was treated with needle knife loosening therapy at conventional position. The two groups were treated for 10 days. The clinical efficacy was observed and the changes in scores of symptoms and signs and pain VAS score were compared. Results ①The total effective rate was 90.00% and 86.67% in the treatment group and the control group, respectively. There was no significant difference in clinical efficacy between the two groups(P>0.05). ②After 5 and 10 days of treatment, the scores of symptoms and signs were higher in the treatment group than in the control group, and the pain VAS score was lower in the treatment group than in the control group(P<0.05). Conclusion The overall efficacy of needle knife loosening therapy at fixed-point rotating position in young patients with cervical spondylotic radiculopathy is comparable with that of therapy at conventional position, but this new method has easy-to-find needle insertion point, shallow needle insertion and high safety, and it causes less damage to normal soft tissues, more easily accepted by patients.
