祛痹汤治疗膝关节骨性关节炎45例

摘　　要：目的:观察祛痹汤治疗膝关节骨性关节炎的临床疗效。方法:将90例膝关节骨性关节炎患者按照随机数字表法分为观察组和对照组,每组各45例。对照组给予常规西药治疗,观察组患者给予祛痹汤治疗。观察两组患者的临床疗效;比较两组患者治疗前后中医证候积分、膝关节功能、膝关节疼痛程度变化积分情况;检测两组患者治疗前后肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-1β(interleukin-1β,IL-1β)、胰岛素样生长因子-1(Insulin-like growth factor-1,IGF-1)、转化生长因子-β(transforming growth factor-β,TGF-β)水平。结果:观察组有效率为93.33%,对照组有效率为77.78%,两组患者有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后中医证候积分、Lequesne评分及VAS评分均低于同组治疗前,且观察组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后TNF-α、IL-1β水平均低于同组治疗前,IGF-1、TGF-β水平均高于同组治疗前;观察组患者治疗后TNF-α、IL-1β水平均低于对照组,IGF-1、TGF-β水平均高于对照组,差异具有统计学意义(P<0.05)。结论:祛痹汤治疗膝关节骨性关节炎,可改善患者的临床症状,促进膝关节功能的恢复。Objective:To observe the clinical curative effect of Bi-Syndrome-Removing Decoction on steoarthritis of knee joint.Methods:Ninety patients with steoarthritis of knee joint were divided into the observation group and the control group according to random number table method,with 45 cases in each group.The control group was treated with conventional Western medicine,while the observation group was treated with Bi-Syndrome-Removing Decoction.Then the clinical efficacies of the two groups were observed,and the scores of TCM syndromes,knee joint function and knee joint pain degree before and after the treatment were compared between the two groups.The levels of tumor necrosis factor-alpha(TNF-α),interleukin-1β(IL-1β),insulin-like growth factor-1(IGF-1),transforming growth factor-beta(TGF-β)were measured before and after the treatment in both groups.Results:The effective rate of the observation group was 93.33%and that of the control group was 77.78%.The difference between the two groups was statistically significant(P0.05).After the treatment,TCM syndrome score,Lequesne score and VAS score of the two groups were lower than those of the same group before the treatment,and the observation group was lower than the control group,and all the differences were statistically significant(P<0.05).After the treatment,the levels of TNF-α and IL-1β in both groups were lower than those before the treatment in the same group,and the levels of IGF-1 and TGF-β were higher than those before the treatment in the same group.After the treatment,the levels of TNF-α and IL-1β in the observation group were lower than those in the control group,and the levels of IGF-1 and TGF-β in the observation group were higher than those in the control group,and all the differences were statistically significant(P<0.05).Conclusion:Bi-Syndrome-Removing Decoction has a remarkable efficacy on steoarthritis of knee joint,can improve clinical symptoms and functional recovery of knee joint.
