中医外治法在恶性肠梗阻治疗中的研究进展

摘　　要：中医治疗肠梗阻以"通"为基本原则,中医外治法治疗恶性肠梗阻以中药穴位外敷、中药保留灌肠、针灸为主要方法。临床应用须辨病与辨证相结合,但这些治疗手段都有局限性,应排除因肿物向内生长堵塞肠腔所致的机械性梗阻,必要时联合西医化疗或姑息性手术解除急症。中医外治法的应用多以经络学说、脏腑学说为理论基础,临床应用时要辨证论治,取穴准确,药物的应用药味也不宜过多,身体羸弱的患者在泻下药为主的前提下酌情配伍行气、化瘀、散结功效的中药可加强肠道反应,也可以防止峻泻过猛损耗正气。因恶性肠梗阻病势凶险,可采用多种外治法协同应用,以期快速达到缓解梗阻的效果。The basic principle of TCM treatment of bowel obstruction is"dredging".The main methods of TCM external treatment for malignant bowel obstruction include external application of acupoints of TCM,preservation enema of TCM,acupuncture and moxibustion.In clinical application,disease differentiation should be combined with syndrome differentiation,but these treatments have limitations.Mechanical obstruction caused by endogenous growth of tumors blocking intestinal cavity should be excluded.If necessary,Western medicine chemotherapy or palliative surgery should be combined to relieve the emergency.The application of external treatment of TCM is based on meridian theory and viscera theory.In clinical practice,syndrome differentiation and treatment should be adopted,acupoints should be selected accurately,and flavors of medicines should not be used too much.Based on purgative medicinals,patients with weak constitution can strengthen intestinal reaction if they are compatible with Chinese medicinals of promoting qi,removing blood stasis and dispersing knots,as well as preventing excessive diarrhea from damaging healthy qi.Because of the serious situation of malignant bowel obstruction,a variety of external therapies can be used together in order to achieve the effect of alleviating obstruction quickly.
