摘  要:目的探讨视频教育在人工全髋关节置换术后功能锻炼中的应用效果。方法选取106例行全髋关节置换患者,按随机数字表法将其分为观察组和对照组,每组53例。观察组,入院第2天开始,每日11:00-12:00组织患者及家属集中观看髋关节健康教育视频,每日循环播放;对照组,常规口头宣教,入院第5天开始,每日上午相同时段观看教育视频。2组其他治疗均相同。比较2组术后功能康复训练的效果、康复训练知识的掌握程度、功能锻炼情况、术后短期并发症及护理满意度。结果术后3个月,观察组Harris评分为(88.34±4.16)分,明显高于对照组(78.49±5.05)分(t=11.063,P<0.05)。观察组术前、术后对锻炼知识的掌握率明显高于对照组[90.6%比71.7%(χ^2=6.163)、92.5%比77.4%(χ^2=4.710),P<0.05]。观察组术后短期并发症发生率明显低于对照组(0.0%比7.5%,χ^2=4.157,P<0.05)。术后7 d,观察组功能锻炼完成正确率高于对照组(98.1%比79.2%,χ^2=9.397、P<0.05)。术后3个月,观察组护理满意度明显高于对照组(96.2%比84.9%,χ^2=3.975、P<0.05)。结论术前视频教育对人工全髋关节置换术后患者的康复效果较传统健康宣教模式明显提升。Objective To investigate the effect of video education on functional exercise after total hip arthroplasty.Methods A total of 106 patients who underwent total hip arthroplasty were randomly divided into two groups,with 53 patients in each group.In observation group,patients and their families watched hip health education video 1 hour per day(11:00-12:00)from the second day after admission.In control group,patients received routine oral education and watched the video from the fifth day after admission.Other treatments were same in both groups.The efficacy of postoperative functional rehabilitation training,mastery of rehabilitation training knowledge,short-term postoperative complications and patient satisfaction with nursing were compared between the two groups.Results At 3 months after operation,compared with control group,patients in observation group showed a significant increase in the Harris score((88.34±4.16)vs(78.49±5.05),t=11.063,P<0.05),as well as in the satisfaction with nursing care(96.2% vs 84.9%,χ^2=3.975,P<0.05).The preoperative and postoperative mastery rates of training knowledge in observation group were higher than those in control group((90.6% vs 71.7%,χ^2=6.163)and(92.5% vs 77.4%,χ^2=4.710),respectively,P<0.05).The incidence of short-term complications in observation group was lower than that in control group(0.0% vs 7.5%,χ^2=4.157,P<0.05).At 7 days after operation,the correct rate of functional exercise in observation group was higher than that in control group(98.1% vs 79.2%,χ^2=9.397,P<0.05).Conclusion Compared with traditional health education,the preoperative video education can obviously improve the efficacy of rehabilitation after total hip arthroplasty.
