摘 要:目的利用256层螺旋CT心脏CTA检查和后处理重建,观察肺血减少型先天性心脏病体肺侧支血管的分布特征。方法回顾性分析37例肺血减少型先天性心脏病并有体肺侧支血管病例的心脏CTA影像资料。结果37例中侧支血管共144支,分别起自颈总动脉、升主动脉、主动脉弓、胸主动脉、头臂干、双侧锁骨下动脉、胸廓内动脉、腹主动脉、椎动脉及肋间动脉;其中进入左肺84支,右肺60支;管腔最粗直径约9.4mm,最细直径约1mm;8例患者并发直径小于1mm的毛细紊乱血管丛;侧支血管管腔局部狭窄5处。结论256层MSCT检查对肺血减少型先天性心脏病体肺侧支血管的检出及其特征的全面显示有其独特作用,其独特的后处理重建技术,特别是MIP能清楚观察毛细MAPCAs的边缘及走行。Objective The distribution characteristics of the pulmonary collateral vessels of the congenital heart disease with reduced pulmonary blood were observed by using 256-slice spiral CT.Methods Cardiac CTA imaging data of 37 cases of pulmonary hypoplasia congenital heart disease with somatic pulmonary collateral vessels were retrospectively analyzed.Results In 37 cases,a total of 144 collateral vessels were involved,starting from the common carotid,ascending aorta,aortic arch,thoracic aorta,brachiocephalic trunk,bilateral subclavian artery,internal thoracic artery,abdominal aorta,vertebral artery and intercostal artery respectively.There were 84 branches of left lung and 60 branches of right lung.The largest diameter of the lumen was about 9.4 mm and the smallest diameter was about 1 mm.Eight patients were complicated with capillary arterial plexus with diameter less than 1 mm.There were 5 local stenosis of collateral vascular lumen.Conclusion The 256-layer MSCT examination has its unique effect on the detection and comprehensive display of the pulmonary collateral vessels of the congenital heart disease with reduced pulmonary blood volume.
