摘  要:目的应用多层螺旋CT三维支气管树图像及薄层横断层图像对右肺下叶支气管分支形式进行分类。方法筛选出298例胸部CT扫描右肺下叶支气管显示良好的病例检查资料,应用多层螺旋CT后处理技术重组右肺下叶支气管的支气管树及薄层横断层CT,综合应用支气管树图像及薄层横断层图像,根据支气管的走行方向、管径粗细及肺野分布范围对右肺下叶支气管的分支类型进行分型。结果应用多层螺旋CT支气管树图像及薄层横断层CT图像,可以将右肺下叶支气管分为5种类型。I型196例(65.8%),为最常见分支类型,表现为右肺下叶支气管依次先后分出背段支气管(B6)、内基底段支气管(B7)、前基底段支气管(B8)、外基底段支气管(B9),最后延续为后基底段支气管(B10)。II型35例(11.7%),表现为右肺下叶支气管依次先后分出B6、B7,最后延续为B8和B10两大主干。III型29例(9.7%),表现为B6、B7依次先后分出,然后延续为两大支,分别是B8与B9的共干支(B8+9)与B10。IV型19例(6.4%),表现为B6分出后,B7未从基底干支气管单独分出,而是基底干支气管分为两大支,分别为B7与B8的共干支(B7+8)及B9与B10的共干支(B9+10)。V型19例(6.4%),B6、B7依次先后发出,然后基底干支气管三分为B8、B9、B10。结论应用多层螺旋CT三维支气管树图像及薄层横断层图像可以对右肺下叶支气管分支形式进行准确分类。Objective To classify the right inferior lobe bronchial branching patterns by 3D bronchial tree and thin-section transverse CT images acquired from multi-slice CT.Methods 298 patients with thorax scans were enrolled.The segmental bronchi were demonstrated in terms of bronchial tree and thin-section transverse CT.Results The segmental bronchial ramifications of the right inferior lobe were classified into five types.Type I(196,65.8%),the superior segmental bronchus(B6),medial segmental bronchus(B7),anterior segmental bronchus(B8),and lateral basal segmental bronchus(B9)arose separately from the right inferior lobe bronchus or basal trunk bronchus.Type II(35,11.7%),B6,B7 arose separately from the right inferior lobe bronchus or basal trunk bronchus,then the basal trunk bronchus bifurcated into B8 and B10.Type III(29,9.7%),B6 and B7 arose separately from the right inferior lobe bronchus or basal trunk bronchus.Then,the basal trunk bronchus bifurcated into common stem of B8 and B9(B8+9),and B10.Type IV(19,6.4%),B6 arose separately from the right inferior lobe bronchus.Then the basal trunk bronchus bifurcated into common stem of B7 and B8(B7+8),and common stem of B9 and B10(B9+10).Type V(29,9.7%),B6 and B7 arose separately from the right inferior lobe bronchus or basal trunk bronchus.Then,the basal trunk bronchus trifurcated into B8,B9 and B10.Conclusion The bronchial branching patterns of the right inferior lobe could be classified with 3D bronchial tree and transverse CT images acquired from multi-slice CT.
