摘　　要：目的探讨前列腺结核的MRI诊断价值。方法回顾性分析10例经病理穿刺或诊断性治疗确诊为前列腺结核的MRI图像,重点分析病变部位、形态、MRI信号特征(包括常规序列T1WI、T2WI、DWI和对比增强)。结果10例患者中3例为局限于前列腺的单发实性小结节,3例累及整个前列腺多发形态不规则结节,4例结核脓肿形成。6例实性结节T1WI呈等、稍低信号,T2WI呈等、低、高信号,DWI呈等、低信号。结核性脓肿脓腔T 1WI呈等、低、略高信号,T2WI呈低、高、混杂信号,DWI呈低、高、混杂信号。脓肿壁及分隔T1WI呈等、稍高信号,T2WI呈稍高信号,DWI呈低信号。增强扫描实性结节中有2例明显均匀强化,4例轻度边缘强化;结核性脓肿壁呈明显厚环形强化3例,轻度边缘强化1例,脓腔无强化。10例患者中伴有泌尿系结核2例、附睾结核1例、精囊腺结核1例、肺结核6例、脑结核5例、脊柱脊髓结核5例。结论前列腺结核的MRI信号多样化,多参数MRI可以对前列腺结核进行早期诊断。Objective To explore the MRI diagnostic value of prostatic tuberculous.Methods The MRI findings in 10 patients of prostatic tuberculous proved by Pathological puncture or diagnostic treatment were retrospectively analyzed.The emphasis was to analyse the lesion location,sharpness,features of signal intensity(including the conventional sequence T1WI,T2WI,diffusion weighted imaging(DWI),contrast enhanced and other parts of the tuberculosis.Results Three in 10 patients were single solid microtubules localized to the prostate,three involved irregular multiple nodules of the prostate and four formed tuberculosis abscesses.6 cases of solid lesions T 1WI was equal,slightly lower signal,T2WI was equal,low,high signal,DWI was equal,low signal.Tuberculous abscess T1WI was equal,low,slightly high signal,T2WI was low,high,mixed signals,DWI was low,high,mixed signals.The abscess wall and separation T1WI were slightly higher signal,T2WI was slightly higher signal,DWI was low signal.In contrast,there were 2 cases of solidified nodules and 4 cases of mild edge enhancement.Tuberculous abscess was significantly thicker ring enhancement in 3 cases,mild edge enhancement in 1 case,abscess no strengthening.Among the 10 patients,there were 2 cases of urinary tuberculosis,1 case of epididymis tuberculosis,1 case of seminal vesicle tuberculosis,6 cases of tuberculosis,5 cases of cerebral tuberculosis,and 5 cases of spinal tuberculosis.Conclusion The MRI signal of Prostate tuberculosis were diversity,the use of multi parameter MRI image can accurately detect the characteristics of prostate diseases,lesions and early diagnosis.
