摘 要:目的观察肠宁汤对急性溃疡性结肠炎(湿热证)大鼠免疫功能、细胞炎症因子及肠道菌群的影响。方法 50只大鼠随机分为空白组、模型组、阳性组、试验组(中药高剂量、低剂量组)。采用饮食加环境配合2,4-二硝基氯苯加醋酸(DNCB)/乙醇复合法制作湿热型大鼠UC模型。阳性组给予美沙拉嗪灌胃,试验组给予肠宁汤灌胃。7 d后观察并比较各组大鼠的结肠组织病理形态学变化及病理评分、免疫功能、细胞炎症因子及肠道菌群变化情况。结果模型组大鼠的疾病活动指数(DAI)、结肠黏膜损伤指数(CMDI)、结肠组织学(HS)评分、CD4+和CD8+、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)和白细胞介素-6(IL-6)水平、大肠杆菌和肠球菌数明显升高,CD3+和CD4+/CD8+、乳杆菌、双歧杆菌数和革兰阳性菌:革兰阴性菌比值明显降低(均P <0.05),菌群严重失调。与模型组比较,阳性组和试验组大鼠的DAI、CMDI及HS评分、CD4+和CD8+、CRP、TNF-α和IL-6水平、大肠杆菌和肠球菌数明显降低,CD3+和CD4+/CD8+、乳杆菌、双歧杆菌数和革兰阳性菌:革兰阴性菌比值明显升高(均P <0.05),菌群失调得到恢复。且中药高剂量组大鼠的DAI、CMDI评分和HS评分、CD4+和CD8+、CRP、TNF-α和IL-6水平明显低于阳性组,CD3+和CD4+/CD8+的明显高于阳性组(P <0.05)。结论肠宁汤能有效地改善急性UC大鼠的病理反应,提高免疫功能,减少细胞炎症因子反应,改善肠道菌群。Objective:To investigate the effects of Changning Decoction on immune function,cytoinflammatory factors and intestinal flora in acute ulcerative colitis rats of dampheat type.Methods:50 rats were randomly divided into the blank group,the model group,the positive group and the experimental groups(high dose and low dose).Wetheat rat UC models were prepared with diet and environment combined with 2,4-dinitrochloroben zene plus acetic acid(DNCB)/ethanol composite method.The positive control group was given mesalazine orally,the experimental group Changning Decoction.After 7 days,the histopathological changes and pathological scores,immune function,cytokines and intestinal flora were observed and compared.Results:DAI,CMDI and HS scores,CD4+ and CD8+,CRP,TNF-α and IL-6 levels,E.coli and Enterococcus were significantly increased in the model group;CD3+ and CD4+/CD8+,the number of Lactobacillus and Bifidobacterium and the ratio of Grampositive bacteria to Gram-negative bacteria decreased significantly,with serious dysbacteriosis(P<0.05).Compared with the model group,those in the positive group and the experimental group decreased significantly;CD3+and CD4+/CD8+,the number of Lactobacillus and Bifidobacterium and the ratio of Grampositive bacteria to Gramnegative bacteria increased significantly,with the restoration of flora imbalance(P<0.05).Those in the high-dose group were significantly lower,while the levels of CD3+and CD4+/CD8+ were significantly higher than those in the positive group(P<0.05).Conclusion:Changning Decoction can effectively improve the pathology and immune function of acute UC rats,reduce cellular inflammatory factor response,and improve intestinal flora.
