全耳内镜下经外耳道上鼓室胆脂瘤手术的临床疗效和安全性

摘　　要：目的探讨全耳内镜下经外耳道上鼓室胆脂瘤手术的临床疗效和安全性。方法纳入2016年1月至2017年12月在我院进行全耳内镜下经外耳道上鼓室胆脂瘤手术的患者共38例,观察患者手术前后的语频区气导听阈、气骨导差。对患者进行随访,观察患者的胆脂瘤复发情况和并发症发生情况。采用中文版慢性耳病调查量表(the Chinese version of the chronic ear survey,CCES)观察患者术前和术后的生活质量。结果患者术后的气导听阈、气骨导差较术前降低,差异具有统计学意义(P<0.05)。患者在随访期间未出现胆脂瘤复发,2例患者随访期间出现真菌性外耳道炎,分别在随访5个月和8个月时出现,并发症发生率为5.26%。患者术后的AR、ST、MR评分和总分均较术前升高,差异具有统计学意义(P<0.05)。结论全耳内镜下经外耳道上鼓室胆脂瘤切除术治疗上鼓室胆脂瘤患者具有显著的临床疗效,可改善患者的预后,提高患者的生活质量,且安全性高。Objective To explore the clinical efficacy and safety of transauditory meatus otoendoscopic surgery for attic cholesteatoma.Methods 38 cases of patients with transauditory meatus otoendoscopic surgery for attic cholesteatoma in our hospital from January 2016 to December 2017 were enrolled.Before and after surgery,the air conduction hearing threshold at speech frequency and air-bone-gap of patients were observed.Patients were followed up to observe the recurrence of cholesteatoma and the incidence of complications.The Chinese version of the chronic ear survey(CCES)was used to observe patients'quality of life before and after surgery.Results The air conduction hearing threshold and air-bone-gap after surgery were significantly lower than those before surgery,with statistical differences(P<0.05).During follow-up period,no recurrence of cholesteatoma was found in all patients,while 2 patients developed fungal otitis externa at 5 and 8 months of follow-up respectively,with the incidence of complications of 5.26%.The scores of AR,ST and MR,and total score of patients after surgery were significantly higher than those before surgery,with statistical differences(P<0.05).Conclusions Transauditory meatus otoendoscopic surgery for attic cholesteatoma has significant clinical efficacy and high safety for the treatment of patients with attic cholesteatoma,which can improve patients'prognosis,and improve patients'quality of life.
