摘 要:目的将急诊危重监护室呼吸机用于急性脑出血合并呼吸衰竭患者中,对其临床价值进行分析。方法对肇庆市第一人民医院2016年2月—2018年3月期间收治的60例急性脑出血合并呼吸衰竭患者分组研究,根据治疗结果分成研究组(n=32,治疗成功)与参照组(n=28,治疗失败),两组均连接呼吸机行辅助呼吸,对两组治疗后血气指标、器官功能情况等进行比较分析。结果两组通气前MAP、pH、PaCO2、PaO2、SaO2各项指标水平相比,差异无统计学意义(P>0.05),研究组通气后pH水平与参照组相比,差异无统计学意义(P>0.05),研究组通气后MAP水平与参照组相比,相对更低(P<0.05),研究组通气后PaCO2、PaO2、SaO2各项血气指标水平与参照组相比,相对更高(P<0.05);两组通气前多器官衰竭、电解质紊乱情况相比,差异无统计学意义(P>0.05),研究组通气后多器官衰竭率为43.75%,与参照组46.88%相比,差异不显著(P>0.05),研究组通气后电解质紊乱率为34.38%,与参照组65.63%相比,相对更低(P<0.05)。结论对急性脑出血合并呼吸衰竭患者实施急诊危重监护室呼吸机辅助呼吸效果理想,可改善血气指标,纠正电解质紊乱,值得应用。Objective To analyze the clinical value of Emergency Critical Care Unit ventilator in patients with acute cerebral hemorrhage complicated with respiratory failure.Methods60 patients with acute cerebral hemorrhage complicated with respiratory failure from February,2016 to March,2018 were divided into study group(n=32,successful treatment)and control group(n=28,failure of treatment).The blood gas indexes and organ function were compared between the two groups after treatment.ResultsMAP,pH,PaCO2,PaO2 and SaO2 before ventilation in both groups were compared.The level of pH after ventilation in the study group was not significantly different from that in the control group(P>0.05).The MAP level in the study group after ventilation was lower than that in the control group(P<0.05).The blood gas levels of PaCO2,PaO2,SaO2 in the study group after ventilation were higher than those in the control group(P<0.05).There was no significant difference in multiple organ failure and electrolyte disorder between the two groups before ventilation(P>0.05).The rate of multiple organ failure after ventilation in the study group was 43.75%,which was46.8%in the control group.The electrolyte disturbance rate after ventilation in the study group was 34.38%,which was lower than that in the control group(65.63%)(P<0.05).ConclusionThe application of ventilator-assisted respiration in Emergency Critical Care Unit for patients with acute cerebral hemorrhage complicated with respiratory failure is effective and can improve blood gas index and correct electrolyte disorder,which is worthy of application.
