摘　　要：目的动态监测中性粒细胞/淋巴细胞比值(NLR)对脓毒症患者的价值.方法采用回顾性研究方法收集韶关市粤北人民医院ICU2018年1-11月明确诊断为脓毒症患者105例,根据脓毒症严重程度分为脓毒症组(33例)、严重脓毒症组(27例)、脓毒性休克组(45例),按治疗结局分为存活组(63例)、死亡组(42例).结果不同程度脓毒症患者相关指标比较发现与脓毒症组比较,严重脓毒症组第1、3、5、7 dNLR,血乳酸(Lac)、降钙素原(PCT)、急性生理与慢性健康状况评分Ⅱ(APACHEⅡ评分)评分差异有统计学意义(P.<0.05);脓毒性休克组中第1、3、5、7 dNLR,T、Lac、PCT、APACHEⅡ评分差异有统计学意义;与严重脓毒症组比较,脓毒性休克组第1、3 dNLR,Lac、APACHEⅡ评分差异有统计学意义(P.<0.05).死亡组在第1、3、5 dNLR、Lac、APACHEⅡ评分显著高于存活组,差异有统计学意义(P<0.05).在预测脓毒症预后的ROC曲线下的AUC分别为第5 dNLR>第3 dNLR>第1 dNLR>Lac>APACHEⅡ评分>T>第7 dNLR.结论动态监测脓毒症患者NLR有助于判断疾病严重程度及评估预后.Objective To dynamically monitor the value of neutrophils/lymphocyte ratio(NLR)in patients with sepsis.Methods105 cases of sepsis in ICU from January to November,2018 in the hospital were retrospectively analyzed. According to the severity of sepsis,they were divided into sepsis(33 cases),severe sepsis(27 cases),and septic shock(45 cases)group. According to the treatment outcome, they were divided into survival group(63 cases)and death Group(42 cases).ResultsCompared with the Sepsis group,the difference of 1 th,3,5,7 dNLR,Lac,PCT and ApacheⅡ scores in severe sepsis group was statistically significant(P<0.05),while in septic Shock group,there was also statistically significant difference of 1 th,3,5,7 dNLR,T,Lac,PCT and ApacheⅡ scores. And compared with severe sepsis group,the 1 th and 3 dNLR,LAC and ApacheⅡ scores of septic shock group were also statistically significant(P<0.05). The scores of death group in 1 th,3,5 dNLR,Lac and ApacheⅡ were significantly higher than those in survival group,and the difference was statistically significant(P<0.05). The AUC under the ROC Curve,which predicted the prognosis of sepsis,was the 5 dNLR>3 dNLR>1 dNLR>Lac>ApacheⅡscores>T>7 dNLR.ConclusionDynamic monitoring of NLR in patients with sepsis is helpful to judge the severity of the disease and evaluate the prognosis.
