摘　　要：目的比较两种手术方法治疗不同体积良性前列腺增生(BPH)的效果。方法将我院收治的200例BPH患者随机分为试验组(腺体体积<80 mL 59例,≥80 mL 41例)和对照组(腺体体积<80 mL 57例,≥80 mL 43例),每组100例。试验组行经尿道等离子前列腺剜切术,对照组行经尿道等离子前列腺电切术。比较两组患者的围手术期情况。结果对于腺体体积<80 mL的患者,试验组的手术时间、膀胱冲洗时间、术后拔管时间及术后住院时间均短于对照组,术中出血量少于对照组(P<0.05)。对于腺体体积≥80 mL的患者,试验组的手术时间长于对照组,术中出血量少于对照组(P<0.05)。结论对于腺体体积<80 mL的BPH患者,应用经尿道等离子前列腺剜切术的手术时间、术后膀胱冲洗时间、术后拔管时间、住院时间更短,术中出血量更少,安全性高。Objective To compare the effects of two surgical methods in the treatment of benign prostatic hyperplasia(BPH)of different volumes.Methods Two hundred BPH patients admitted in our hospital were randomly divided into experimental group(59 cases with prostatic volume<80 mL and 41 cases with prostatic volume≥80 mL)and control group(57 cases with prostatic volume<80 mL and 43 cases with prostatic volume≥80 mL),with 100 cases in each group.The experimental group underwent transurethral plasma kinetic enucleation of prostate,while the control group underwent transurethral plasma kinetic resection of prostate.The perioperative conditions of the two groups were compared.Results For patients with prostatic volume<80 mL,the operation time,bladder irrigation time,extubation time and hospitalization time of the experimental group were shorter than those of the control group,and the intraoperative bleeding volume was less than that of the control group(P<0.05).For patients with prostatic volume≥80 mL,the operation time of the experimental group was longer than that of the control group,and the intraoperative bleeding volume was less than that of the control group(P<0.05).Conclusion For BPH patients whose prostatic volume is less than 80 mL,the operation time,bladder irrigation time,extubation time and hospitalization time of transurethral plasma kinetic enucleation of prostate are shorter,the intraoperative bleeding volume is less and the safety is higher.
