乌司他丁联合腔镜手术固定在连枷胸伴肺挫伤患者中的应用效果

摘　　要：目的探讨乌司他丁联合腔镜手术固定在连枷胸伴肺挫伤患者中的应用效果。方法选取我院收治的81例连枷胸伴肺挫伤患者,使用抽签方式将其分为对照组(39例,腔镜手术固定)和研究组(42例,腔镜手术固定+乌司他丁)。比较两组不同时期的炎性因子水平、住院时间及成人呼吸窘迫综合征(ARDS)的发生情况。结果D4、D7时,两组的TNF-α、IL-6水平均显著高于D1时,D7时显著低于D4时(P<0.05);D4、D7时,研究组的TNF-α、IL-6水平显著低于对照组(P<0.05);研究组的住院时间显著短于对照组,ARDS的发生率显著低于对照组(P<0.05)。结论乌司他丁联合腔镜手术固定治疗连枷胸伴肺挫伤患者可减轻炎性反应,缩短住院时间,减少ARDS的发生。Objective To explore the application effect of ulinastatin combined with laparoscopic surgery fixation in patients with flail chest and pulmonary contusion.Methods A total of 81 patients with flail chest and pulmonary contusion were selected and divided into control group(39 cases,laparoscopic surgery fixation)and study group(42 cases,laparoscopic surgery fixation+ulinastatin)by drawing lots.The levels of inflammatory factors at different times,hospitalization time and the occurrence of adult respiratory distress syndrome(ARDS)were compared between the two groups.Results The levels of TNF-α、IL-6 at D4 and D7 time in both groups were significantly higher than those at D1 time,those at D7 time were significantly lower than D4 time(P<0.05).At D4 and D7 time,the levels of TNF-α、IL-6 in the study group were significantly lower than those in the control group(P<0.05).The hospitalization time of the study group was significantly shorter than that of the control group,the incidence of ARDS was significantly lower than that of the control group(P<0.05).Conclusion Ulinastatin combined with laparoscopic surgery fixation in patients with flail chest and pulmonary contusion can reduce inflammatory response,shorten the hospitalization time and reduce the occurrence of ARDS.
