子宫内膜异位症合并不孕腹腔镜术后中医干预方案的探讨

摘　　要：目的:观察轻度子宫内膜异位症(内异症)合并不孕宫腹腔镜术后在月经周期的不同时机采用中药内服结合保留灌肠干预的临床疗效。方法:将符合纳入标准的90例轻度内异症合并不孕宫腹腔镜术后的患者,采用随机数字表分为A组(经净后干预20天)、B组(经净后干预10天)和C组(经前干预10天),每组30例,比较3组治疗后的子宫内膜厚度、回声类型、子宫动脉血流情况、宫颈黏液评分、周期排卵率和妊娠率。结果:3组患者子宫内膜平均厚度、内膜类型、PI及RI比较,差异均有统计学意义(P<0.05)。子宫内膜平均厚度组间比较除A组与B组差异无统计学意义外(P>0.05),其余两组均有统计学意义(P<0.05)。3组患者宫颈黏液评分、周期排卵率及妊娠率比较,差异均有统计学意义(P<0.05)。结论:轻度内异症合并不孕宫腹腔镜术后采用中药内服结合保留灌肠经净后干预20天疗法,可提高子宫内膜容受性,获得较好的临床疗效。Objective:To observe the clinical efficacy of mild endometriosis combined with infertility intervened by oral administration of Chinese herbal medicine combined with retention enema after laparoscopic surgery at different time of menstrual cycle.Methods:90 cases of mild endometriosis with infertility after laparoscopy were randomly divided into group A(20 days of intervention after menstruation),group B(10 days of intervention after menstruation)and group C(10 days of intervention before menstruation),with 30 cases in each group.The thickness of endometrium,the echo type,uterine artery blood flow,cervical mucus score,cycle ovulation rate,pregnancy rate were compared among the three groups after the treatment.Results:There were statistical differences in the average thickness of endometrium,endometrial types,PI and RI among the three groups(P<0.05).There was no significant difference in endometrial mean thickness between group A and group B(P>0.05),but every other two groups had statistical significance(P<0.05).There were statistically significant differences in cervical mucus score,cycle ovulation rate and pregnancy rate among the three groups(P<0.05).Conclusion:The intervention of oral administration of Chinese herbal medicine combined with retention enema 20 days after menstruation can improve the endometrium receptivity and increase clinical effect in the treatment of mild endometriosis with infertility after laparoscopic surgery.
