摘　　要：目的探讨感染科发热患者血培养阳性结果的预测因素。方法回顾性分析556例血培养患者的临床资料及血培养结果,采用多因素Logistic回归分析血培养阳性结果的预测因素,采用受试者工作特性(ROC)曲线分析相关因素的预测价值。结果Logistic回归分析结果显示,C-反应蛋白(OR=1.006)、降钙素原(OR=1.020)、白细胞计数(OR=1.108)和序贯性器官功能障碍评分(OR=1.656)与血培养阳性结果正相关(P<0.05);联合4项因素进行血培养阳性结果预测,AUC为0.816。结论白细胞计数、C-反应蛋白、降钙素原和序贯性器官功能障碍评分是预测发热患者血培养阳性的相关因素,可综合应用上述因素来指导血培养采集。Objective To identify the predictive factors that leading to positive blood culture of the febrile patients in infectious disease ward.Methods Firstly,the clinical data of 556 febrile patients as subjects of blood culture and the results of culture were retrospectively analyzed.Secondly,multivariable logistic regression analysis was conducted to map the potential predictive factors.Receiver operating characteristic(ROC)curve was then used to evaluate the factors’predictive efficacy.Results The logistic regression analysis indicated that C-reactive protein(OR=1.006),procalcitonin(OR=1.020),white blood cell count(OR=1.108)and sequential organ failure assessment scores(OR=1.656)were positively correlated to positive blood culture(P<0.05).The area under the ROC curve(AUC)for the combined use of the four factors was 0.816.Conclusions White blood cell count,C-reactive protein,procalcitonin,and sequential organ failure assessment are the predictive factors for positive blood culture.Therefore,these factors can serve as reference for the practice of blood culture.
