摘　　要：目的通过三维彩色多普勒超声技术,测量子宫内膜血流动力学参数,探讨该方法在预测冷冻胚胎移植妊娠结局中的价值。方法选取2017年11月至2018年4月生殖中心接受冷冻胚胎移植的患者261例,入组标准为:HCG日或内膜转化日内膜厚度均8~14 mm,形态A或A-,均移植2个8细胞Ⅰ~Ⅱ级卵裂期胚胎。排除子宫内膜厚度或形态不达标准、子宫内膜炎、内膜息肉、黏膜下肌瘤、宫腔粘连、剖宫产憩室、输卵管积水及子宫腺肌症的患者。测量其HCG日或内膜转化日子宫内膜厚度、容积、形态、内膜血管化血流指数(VFI)。结果临床妊娠组HCG日或内膜转化日内膜VFI高于非临床妊娠组(P<0.05)。VFI的ROC曲线下面积为0.605,其CUTOFF值为3.55。结论 HCG日或内膜转化日测量子宫内膜VFI对预测冷冻胚胎移植结局是有价值的。Objective To investigate the application value of three dimensional color Doppler in predicting the pregnancy outcomes of frozen embryo transfer.Methods A total of 261 patients aged 25~40 years who underwent frozen embryo transfer in our hospital from November 2017 to April 2018 were enrolled in the study. The criteria of enrolling into the study were as follows:endometrial thickness 8~14 mm on HCG day or endometrial transformation day, morphous A or A-, 2~8 cells at stage I^II cleavage embryos. The patients whose endometrial thickness or morphology did not meet the standards and those with endometritis,endometrial polyps,submucosal fibroids,intrauterine adhesions,history of cesarean section,hydrosalpinx,and adenomyosis were excluded from the study. The endometrial thickness,volume, morphous and endometrial vascularization flow index(VFI) on the HCG day or endometrial transformation day were measured.Results The endometrium VFI on HCG day or endometrial transformation day in clinical pregnancy group was significantly higher than that in non clinical pregnancy group(P<0.05).The area under the ROC curve of VFI was 0.605,and its CUT OFF value was 3.55.Conclusion Measurement of endometrial VFI on HCG day or endometrial transformation day is of great value in predicting the outcome of frozen embryo transfer.
